L-96-315AttachmentRe1toaNoticeofViolationB)TheInstrumentandControldepartmentsupervisorsandtechnicianswerebriefedonthiseventandontherequirementsofQI12-PR/PSL-1"CalibrationofMeasuringandTestEquipment"andQI12-PR/PSL-2"ControlandCalibrationofMeasuringandTestEquipment".4.CORRECTIVESTEPSTOAVOIDFURTHERVIOLATIONSA)QualityInstructionQI12-PR/PSL-2wasrevisedinAugust,1996,tostatethat"BorrowingofM&TEinthefieldisnotanacceptablepractice."ThisQualityInstructionandtheaboveviolationarebeingincludedinthefourthquarterI&CIndustryandIn-houseEventstrainingprogramtofurtheremphasizeMATEcontrolrequirements.B)OnSeptember17,1996,amemorandumwasissuedtoallmaintenancepersonnel(MGMT/PSLLtr.Bk.¹96-102)whichinstitutedadditionalcontrolsforaccesstoM&TE.TheadditionalcontrolsincludelockingofMATEstorageareasandaccessrestrictionstoareasinwhichM&TEisstored.TherequirementtoensurethatallM&TEisproperlyassignedtoaspecificNuclearPlantWorkOrderwasemphasized.5.FullcompliancewasachievedonNovember15,1996,whentheM&TElogwasupdatedtoincludetheuseofmegohmeterPSL-865toperformworkorder95011787(NPWO4546/63).ViolationB:TechnicalSpecification6.8.1.arequiresthatwrittenproceduresbeestablished,implemented,andmaintainedcoveringtheactivitiesrecommendedinAppendixAofRegulatoryGuide1.33,Revision2,February,1978.AppendixA,paragraphl.dincludesadministrativeproceduresforproceduraladherence.ProcedureQI5-PR/PSL-1,Revision73,"Preparation,Revision,Review/ApprovalOfProcedures,"Section5.14.1requiresverbatimcompliancetoproceduresbyallpersonnel.Inaddition,step5.14.4.A.3requiresthatprocedurescontainingtaskswhichmustbeperformedinaspecifiedsequenceand/orwhichverificationisdocumentedbyinitialorsignaturemustbepresentandreferredtodirectlyduringtheperformanceoftheactivity.Contrarytotheabove,onSeptember14,1996,ahealthphysicstechnicianperformingagrabsampleoftheUnit1containmentfailedtohaveProcedureHPP-22,Revision4,"Air L-96-315AttachmentRe1toaNoticeofViolationSampling",AppendixA,inhand,whileitwasbeingperformed.Asaresult,step7.21ofthisprocedurewasnotperformedwhichrenderedthecontainmentradiationmonitorinoperable.ThisisaSeverityLevelIVviolation(SupplementI).~ResonseB:1.FPLconcurswiththeviolation.2.REASONFORTHEVIOLATIONThecauseofthisviolationwascognitivepersonnelerroronthepartofaHealthPhysicsRadiationProtectionMan(RPM)performingagrabsampleoftheUnit1containmentatmosphereinpreparationforacontainmententrytosupportInstrumentandControldepartmentworkactivities.ThesamplewasbeingtakenviatheUnit1ParticulateIodineGasmonitorinaccordancewithhealthphysicsprocedureHPP-22"AirSampling".Specifically,theRPMperformingthesampledidnothaveprocedureHPP-22inhandasrequiredwhenrealigningthemonitortonormaloperation.TheRPMfeltthathehadthestepsoftheprocedurememorized.Thisledtomissingakeystepintheprocedure,whichresultedinthemonitorbeingdeclaredbackinservicewiththethrottlevalveintheincorrect(closed)position.3.CORRECTIVESTEPSTAKENANDTHERESULTSACHIEVEDA)Upondiscoverythatthecontainmentradiationmonitorwasinoperable,theRPMplacedthethrottlevalveinthecorrectpositionandtheSeniorNuclearPlantOperatorverifiedthatthemonitorwasbackinservice.ThisactionwascompletedonSeptember14,1996.B)HealthPhysics(HP)managementperformedaninvestigationoftheeventandconcludedthattheeventwascausedbypersonnelerror.DisciplinaryactionwasdeterminedasappropriateforthesituationandadministeredinaccordancewithFPLpolicy.Theindividual'sRPMqualificationwassuspendeduntilsuccessfulremedialtrainingcouldbecompleted.C)HealthPhysicsprocedureHPP-22wasreviewedbyHPmanagementpersonnelforadequacyandpotentialhumanfactorsissuesthatmayhavecontributedtotheeven L-96-315AttachmentRe1toaNoticeofViolationItwasconcludedthatuseoftheprocedurewouldhavebeenadequatetopreventtheaboveviolation.D)StanddownmeetingswereconductedwithHPpersonneltodiscusstheeventandreviewtherequirementsofHPP-22.Therequirementtohavetheprocedureinhandwhileperformingthetaskwasstressed.4.CORRECTIVESTEPSTOAVOIDFURTHERVIOLATIONSA}HealthPhysicsProcedureHPP-22wasrevisedtoemphasizetherequirementtoperformIndependentVerificationforcorrectvalvepositioningttriortodeciaringthemonitorbackinservice:.HPP-22istheonlyHealthPhysicsprocedurethatinvolvestakingapieceofequipmentoutofservice.ThisprocedurechangewascompletedandissuedonSeptember27,1996.B)TheRPMinvolvedintheeventsuccessfullycompletedaremedialtrainingprogramandwasreinstatedasanRPMonNovember22,1996.Theremedialtrainingconsistedofproceduralstudy,awrittenexam,anoralexam,andon-the-jobtrainingwithqualifiedpersonnel.Thiswasfollowedbyaface-to-facemeetingwiththeSiteVicePresidentconcerningproceduraladherenceandattentiontodetail.C)HPpersonnelreceivedadditionaltrainingontherequirementsofHPP-22.Thetrainingstressedtherequirementtohavetheprocedureinhandwhenperformingthetaskandtheexpectationofverbatimcompliance.ThistrainingwascompletedonNovember12,1996.FPLiscontinuingtoreinforceacultureofstrictcompliancewithproceduresandattentiontodetail,andiscontinuingtoconductperiodicstanddownmeetings,whenappropriate.5.FullcompliancewasachievedonSeptember14,1996,withthecompletionofitem3A,above.
