St.LucieUnits1and2DocketNos.50-335and50-389ReplytoaNoticeofViolationVIOLATION!10CFR50AppendixR,"FireProtectionProgramforNuclearPowerFacilitiesOperatingPriortoJanuary1,1979",CriterionIII,"SpecificRequirements",paragraphH,"FireBrigade",required,inpart,that:"Thequalificationofthefirebrigademembersshallincludeanannualphysicalexaminationtodeterminetheirabilitytoperformstrenuousfirefightingactivities".Contrarytotheabove,tenfirebrigademembersfailedtocompleteanannualphysicalexamination.Thesefireteammemberswereassignedforatotalof61shiftfireteampositionsinApril,1996.ThisisaSeverityLevelIVviolation(SupplementI)ItRESPONSEcFPLconcurswiththeviolation.St.LuciePlantdidnothaveeffectiveprocessesforensuringthatspecialqualificationrequirementsforcertainemergencyresponsepersonnelwerecurrent.G.neralassignmentofpersonneltothefireteamwasbymetnsofthemonthlyEmergencyRoster,whichispublishedbythePlant'sTrainingDepartment.Themonthlyrosterwasbasedonfireprotectiontraininganddidnotaccountforphysicalexams,respiratorfits,andrespiratorphysicals.Aseparatesystem,RadiationExposureMonitoringandAccessControlSystem(REMACS),existedfornotifyingOperationssupervisionofthestatusoftheseadditionalqualifications.TheNuclearWatchEngineer(NWE),whoisresponsibleforassuringtheadequacyofstaffingbyon-shiftpersonnelforthefireteamduties,failedtoperformth'isfunction.ContributingtothisfailureofindividualperformancewasthefactthattherewasnochecklisttopromptacrosscheckoftheREMACSprintoutagainsttheEmergencyRosterpriortopersonnelbeingassignedtothefireteam.Consequently,hedidnotproperlyconfirmallapplicablequalificationswerecurrentpriortoassigningon-shiftpersonneltothefiretea '~0 St.LucieUnits1and2DocketNos.50-335and50-389ReplytoaNoticeofViolation3~A.B.C.TheresponsibleNWEwascounseledanddisciplined.Atthetimetheviolationwasidentified,theshiftcomplementof,licensedandnon-licensedoperators(includingfireteam)wasreviewedtoassurethatassignedindividualsmetallqualificationrequirements.Physicalexaminationsandsupportingdocumentationforoperatorrespiratorqualificationswerereviewedandverifiedtobeaccurateandcurrent.D.IThedocumentationforSelfContainedBreathingApparatus(SCBA)trainingwasreviewed.Thetrainingrecordsforsevenindividualsintheoperationsdepartmentwerenotretrievable.Theseindividualsweresubsequentlyretrained.E.TheOperationsSchedulerhasbeenassignedtheresponsibilityandaccountabilityoftrackingandensuringmaintenanceofgeneralqualifications(suchasRCAT,GET,respiratorfit,respiratorphysicals).A.B.C.D.TheEmergencyRosterformatwillberevised,byAugust1,1996,toincludeOperationsDepartmentqualifications.TheEmergencyRosterwillbepublishedbytheSt.LuciePlantTrainingDepartmenteachmonthandwilllisttherequiredqualificationsforthepositions,aswellastheindividualswhoarequalifiedtofillthosepositions.PlantadministrativeprocedureAP0005729,FireProtectionTraininy,QualificationandRequalification,wasrevisedtorequirenotificationofon-shiftOperationsmanagementofthefailureofaphysicalexamination,orfailureofFireBrigadetraining/requalificationbyOperationsDepartmentpersonnel.HealthPhysicsproceduresHP-73,PortacountPlusFitTestSystem,andHPP-60,RespiratoryProtectionManual,wererevisedtorequirenotificationofon-shiftOperationsmanagementofthefailureofarespiratorqualificationbyOperationsDepartmentpersonnel.TheOperationsSupervisorissuedaNightOrderreemphasizingthepersonalresponsibilityofeachindividualintheOperationsDepartment,asstatedin

St.LucieUnits1and2DocketNos.50-335and50-389ReplytoaNoticeofViolationOperationsPolicyOPS-202,MaintainingQualification,tomaintainactivequalificationsandtobecurrentlyqualifiedforthepositionswhichtheyarefillingwithintheplantorganization.5.FullcompliancewasachievedonMay7,1996,whentheexistingandon-comingshiftcomplementsofoperators(includingfireteam)werereviewedandassignedtoshiftresponsibilitieswithconfirmedcurrentphysicalexaminationqualifications.
