On September 30, 2023, the U.S. Nuclear Regulatory Commission (NRC) completed an inspection at Beaver Valley Power Station, Units 1 and 2. On October 18, 2023, the NRC inspectors discussed the results of this inspection with Barry Blair, Site Vice President, and other members of your staff. The results of this inspection are documented in the enclosed report.

No findings or violations of more than minor significance were identified during this inspection.

SUMMARY

The U.S. Nuclear Regulatory Commission (NRC) continued monitoring the licensees performance by conducting an integrated inspection at Beaver Valley Power Station, Units 1 and 2, in accordance with the Reactor Oversight Process. The Reactor Oversight Process is the NRCs program for overseeing the safe operation of commercial nuclear power reactors. No findings or violations of more than minor significance were identified.

PLANT STATUS

Unit 1 started the inspection period at full power. On September 15, 2023, the unit was downpowered to 48 percent power to repair the 'B' main feedwater pump. Following repairs, Unit 1 returned to full power on September 22, 2023. Unit 1 remained at full power for the remainder of the inspection period.

Unit 2 started the inspection period at full power. On the morning of August 14, 2023, the unit was downpowered to perform a planned 'C' steam generator water level instrument calibration, and returned to full power level the same afternoon. Unit 2 remained at full power for the remainder of the inspection period.

INSPECTION SCOPES

Inspections were conducted using the appropriate portions of the inspection procedures (IPs) in effect at the beginning of the inspection unless otherwise noted. Currently approved IPs with their attached revision histories are located on the public website at http://www.nrc.gov/reading-rm/doc-collections/insp-manual/inspection-procedure/index.html. Samples were declared complete when the IP requirements most appropriate to the inspection activity were met consistent with Inspection Manual Chapter (IMC) 2515, Light-Water Reactor Inspection Program - Operations Phase. The inspectors performed activities described in IMC 2515, Appendix D, Plant Status, observed risk significant activities, and completed on-site portions of IPs. The inspectors reviewed selected procedures and records, observed activities, and interviewed personnel to assess licensee performance and compliance with Commission rules and regulations, license conditions, site procedures, and standards.

The inspectors evaluated the ability of the facility licensee to safeguard examination material, such that the examination is not compromised.

Remedial Training and Re-Examinations

The inspectors evaluated the effectiveness of remedial training conducted by the licensee, and reviewed the adequacy of re-examinations for licensed operators who did not pass a required requalification examination.

Operator License Conditions

The inspectors evaluated the licensees program for ensuring that licensed operators meet the conditions of their licenses.

Control Room Simulator

The inspectors evaluated the adequacy of the facility licensees control room simulator in modeling the actual plant, and for meeting the requirements contained in 10 CFR 55.46.

INSPECTION RESULTS

Minor Performance Deficiency 71152A Instrument, Relay, Switch Performance Trending Minor Performance Deficiency

Minor Performance Deficiency: The inspectors selected instrument and relay performance trending as a focused review issue based on industry operating experience and observations that some BVPS instruments experienced drift outside of the Measured Maintenance Database Program limits. Inspection scope included a review of periodic calibration results, issue documentation and prioritization, causal evaluation, and corrective actions for 13 instruments and relays which experienced repetitive drift during periodic calibrations since 2007. The inspectors also performed plant walkdowns, interviewed engineers and operators, and reviewed setpoint calculations and the measured maintenance database calibration database to assess various instrument and relay performance.

The inspectors determined instruments and relays were typically calibrated at the normal intervals, which is a performance deficiency. Notwithstanding, the inspectors identified that since 2007, Unit 2 volume control tank level instrument 2CHS-LT115 calibration was consistently scheduled and performed late in the preventative maintenance grace window or beyond the preventative maintenance late date. The average calibration interval was two years and seven months, which exceeded the 2-year instrument calibration periodicity plus 25 percent grace period. Consequently, as-found instrument drift typically exceeded the Measured Maintenance Database Program drift limit until 2CHS-LT115 was replaced on August 29, 2022. Engineers initiated CRs 2023-06765 and 2023-07052 to document the untimely calibration issue, identify the extent of condition, and restore timely calibrations.

Screening: The inspectors determined the performance deficiency was minor. The inspectors determined untimely calibration of 2CHS-LT115 was a minor performance deficiency because the safety injection actuation circuit was operable and was credited by technical specifications to automatically swap the charging pump suction from the volume control tank to the refueling water storage tank upon sensing a significant reactor coolant system leak, independent of 2CHS-LT115. Additionally, operators performed a channel check surveillance between the two volume control tank level instruments (2CHS-LT115 and 2CHS-LT112) every 12 hours1.388889e-4 days <br />0.00333 hours <br />1.984127e-5 weeks <br />4.566e-6 months <br />, which would identify any significant instrument drift in a timely manner.

Observation: Radiation Monitoring Equipment Reliability 71152S The inspectors performed an in-depth review of Energy Harbors analysis and corrective actions associated with CR-2022-08430, dated November 4, 2022. The inspectors also reviewed eight other recent corrective action documents addressing similar issues and the corrective actions taken or planned to increase the reliability of the radiation monitoring equipment. After reviewing the corrective action documents, the inspectors noted that the backlog of documented radiation monitor issues continues to increase in examples; however, no consequential impacts to the risk significant radiation monitors needed for the Emergency Plan or Technical Specifications were found.
