Inspection Report No. 50-333/85-12 We acknowledge receipt of your July 23, 1985 letter which transmitted your response to the Notice of Violation enclosed with our June 21, 1985 letter.
Thank you for informing us of the corrective and preventive actions documented in your letter.
These actions will be examined during a future inspection of your licensed program.
Regarding your response to Items A and B of the Notice of Violation, based on an August 2,
1985 telephone conversation between Mr.
E.
Mulcahey of your staff and Mr. R. Nimitz of this office, we understand that the following actions have been or will be undertaken:
o an inventory of Master Radiation (MR) keys issued prior to April
1985 will be performed by September 30, 1985.
This inventory will be performed to-determine if individuals, without proper
'
authorization possess MR keys. Appropriate action will be taken to collect and/or reissue improperly issued MR keys.
o a detailed procedure (Standing Order) for control and issue of MR keys will be established and implemented by September 1, 1985.
O the instructions for supervisory oversight of radiologically significant work activities will be included in the Radiation Protection Manual.
This manual is expected to be finalized by January 1,1986 and will be implemented by February 28, 1986.
A description of radiologically significant work will be included in the manual.

Operations personnel have been reinstructed in revised operations i
procedures for key audits and the need to adhere to such procedures via the Night Order program.
If your understanding of the above is not consistent with ours, please notify us within 25 days of receipt of this letter.

In accordance with the provisions of 10 CFR 2 Appendix C, we are submitting our response to Appendix A Notice of Violation trans-

mitted by your letter dated June 21, 1985, as received by the undersigned on June 28, 1985.
This refers to the inspection conducted by Mr. R. L. Nimitz of your office on April 22 through 26, 1985, at the James A. FitzPatrick Nuclear Power Plant.
RESPONSE.TO NOTICE OF VIOLATION A.
The Power Authority agrees with this finding:
The fundamental cause of this violation was that plant workers did not thoroughly read RWP requirements and leadmen did not ensure that their work crews comply with each and every aspect.
l Immediate Corrective Action:
On April 25, 1985 a memo was distributed to plant workers reminding them of their responsibility for complying with RWP requirements and Radiation Protection Procedures in general. This memo (JSOP-85-037) also reminded RES super-d visory and technical personnel of their responsibility for verifying worker RWP compliance.

counseled on the importance of obtaining ALARA review Qv signatures in a timely manner.
It should be noted that y
this is an isolated incident in that 71 ALARA reviews have been completed or are in progress and only one was found lacking a required review signature.
Permanent Corrective Action:
Work practice guides are being developed to provide RES technical personnel with formal guidance when monitoring more radi ignificant work activities.
