This inspection effort involved a total of 31 inspector-hours by one.NRC inspector.

Licensee Action on Previously Identified Items (Closed) Noncompliance (50-330/79-13-01). During the reporting period, the Resident Inspector reviewed the.iydrostatic Test Report for the retect of Unit 2 Incare Instrument Tank 2T-87.

The supplemental test was performed

and witnessed by the Resident Inspector on May 16, 1979. However, the test'

report was not made available by Chicago Bridge and Ir<an until a later date.

The supplemental test report was signed as being acceptable by the Hartford-Boiler Authorized Inspector on August 22, 1979.

The Resident Inspector reviewed selected portions of the quality verifica-tion documentation associated with the licensee accep tance of the Incore Instrument Tanks. This documentation indicated that audits were performed by Consumers Power Co. and Bechtel Power Corp.land tSat the requirements of Project Quality Control Instructions No..PQCI-SM 1.17 Rev. O for erection-of Incore Service Tanks had been accomplished.

Cons,truction Quality Control Release No. QCFM-8807 for the Incore Instrument. Tanks was signed and dated-December 8,1980, and authorized turnov er and acceptance of the tanl c.

Consumers Power Co. Nonconformance Report No. M-01-5-9-037 pertaining to inability to. inspect the tanks was satisfactorily closed on September' 17, 1979.

The review of the quality documentation revealed that drawing number 7220-M-90-20, Support Rings for Incore Instrumentation Tanks, was clari-fled with Revision 5 to permit field cut of a maximum radius " rat hole" at the vertical weld seams for accessibility. Visual Examination Report No. 1, R-3 (1,2,3) indicated that vertical welds 1A, IB, 2A, 2B, 3A and 3B for tank 2T-87 had been visually inspected during tank fabrication.

Liquid Penetrant Teat Report No. 4, (NCL #6) indicated that attachment locations in proximity to vertical weld 1A and the intersection of the support ring satisfactorily passed a penetrant test examination after removal of the temporary attachment. A nominal 3/4 inch long area of vertical seam is obstructed by the support ring. The licensee stated

that a subsequent review of radiographic film indicated that vertical seam lA of tank 2T-87 had been radiographed to within 15/16 inch of the area in question and that no defects had been detected.

Based on the above reviews and efforts of the licensee, the item of noncompliance referenced in Appendix A of NRC Inspection Report No.

50-329/79-13; 50-330/79-13 is considered closed and no further action is considered at this time.

Functional or Program Areas Inspected 1.

Consumers Power Audits of The Zack Co.

During the reporting period, the Resident Inspector reviewed audit reports for audits of the Zack Co. performed on Nov. 18-24, Dec. 2-4, and Dec. 3, 1980. The audits covered Zack Co. activities at the Midland Site, Zack Co. activities at the Chicago offices and Bechtel Supplier Quality Representative (SQR) activities at the Zack Co.

Chicago of fice; respectively. The audits were performed by Consumers Power Co.

The audit of the Zack Co. activities at the Midland Site resulted in

five audit findings and two items requiring more information to assess compliance with requirements (Unresolved Items). One of the Unresolved items, pertaining to missing signatures and/or information on two Plant Bulk-Material Travelers, resulted in an audit finding. The five audit findings require corrective action and a subsequent response.

The audit of the Zack Co. activities at the Chicago offices resulted in seven audit findings.

Four of the audit findings were issued as being resolved and require no further action. The three remaining items rt: quire corrective action and a response. One of these items, pertaining to duct segment leak testing, resulted in Consumers Power Co. issaing a formal Midland Project QA Stop Work Order on Dec. 8,1980.

The Duct Segment Leak Testing Procedure " requires that pressure class 5 duct be tested at + or -2" w.g. or 1 1/2 times the specified opera-ting pressure, whichever is greater. The audit finding indicated that the required test pressure nor the actually applied test pressure com-plied with the requirements of Technical Specification M-151A, HVAC Seismic Class I, Equipment and Duct-work i..stallation. This specifi-ca tior. requires that positive pressure duct be tested with positive

pressure and negative pressure duct be tested with negative pres-The licensee found this pressure test discrepancy to exist sure.

in all of the seventeen duct pieces examined for conformance to test specification requirements. Additionally, allowable 1erkage was not determined as per the technical specifications for tha seventeen duct pieces referenced above.

The audit of the Bechtel Supplier Quality Representative activities resulted in one Unresolved Item pertaining to the lack of availa-bility for review of certification documentation for the assigned SQR at the Zack Co. offices in Chicago.

Exit Interview The Resident Inspector met with licensee representatives (denoted under Persons Contacted) on December 29, 1980. The inspector summarized the scope and findings of the inspection effort to date. The licensee acknowledged the findings reported herein.
