Licensee Action on Previous Inspection Findings (0 pen) Noncompliance Item (50-329/78-03-03; 50-330/78-03-03): The NRC inspector, accompanied by licensee representatives, selectively rein-spected the welds in accessible areas in the cable spreading room, which were reported to have been repaired.

It was determined that the welds were found to be acceptable. The licensee was unable to produce records to indicate that the welds were reinspected. The licensee stated that repaired welds will be reinspected and documented in inspection reports.

The NRC inspector will review the results of reinspection during a future inspection.

(Closed) Unresolved Item (50-329/80-01-05; 50-330/80-01-05): During a previous inspection, it was determined that unapproved drawings were utilized to install instrument tubing for safety-related flow transmit-ters. The licensee documented this matter in a NCR No. M-01-04-0002.

Paragraph 5.3.2 and Section 8 of Specification J218(Q) originally re-quired the completed drawings to be submitted "for review, analysis and-2-

t approval by project engineer prior to installation". During the current inspection, the NRC inspector determined that Specification Change Notice No. 7 dated February 15, 1980 was issued deleting the requirements "for approval prior to installation". The NRC inspector was informed 3 hat the licensee elected to review the drawings concurrently while the installa-tion is in progress in lieu of obtaining approval of drawings prior to installation.

For this purpose, a Resident Assistant Project Engineer has been designated to review and approve drawings on behalf of Bechtel Ann Arbor engineering management. The licensee stated that the intent of the concurrent review by the resident assistant project engineer on site is to incorporate appropriate changes in a timely manner and preclude incorrect installations. The NRC inspector informed the licensee that there are no further questions on this matter and that the progress of the instrument tubing will be reviewed during future routine inspections.

Review of Surveillances on Instrument Tubina Installation The inspector determined that the licensee plans to perform an

audit on the instrument tubing installation activities during June / July 1980. The licensee performed surveillances on these

activities and documented findings in Nonconformance Reports (NCRs). Bechtel QA monitored the installation activities and documented their findings in monitoring records.

The inspector selectively reviewed the following NCRs:

(1) NCR No. M-01-4-9-070 dated July 18, 19.79, identified that split ring washers were used for installing manifold mountings instead of flat washers.

Corrective action

taken was to remove the split ring washers and replace them with flat washers. Corrective action was verified and the NCR closed on November 16, 1979.

(2) NCR No. M-01-4-9-101 dated August 24, 1979 identifies that the installation records of thermowells No. 2TE-1958 and 2TE-1957 indicated that they were inspected to isometric drawing M-619 Sheet 2, Revision 6/F5 instead of M-619 Sheet 3, Revision 6/F2. Corrective action was to rein-spect the thermowells and correct the inspection records.

Corrective action was verified and the NCR closed on November 12, 1979.

(3) NCR No. M-01-2-9-039 dated April 12, 1979 identifies discrepancies in the documentation supplied by Parker-Hannafin Corporation. Corrective action was taken to obtain recertification and the NCR was closed on August 18, 1979.

The inspector reviewed a record of a monitoring activity per-formed by Bechtel QA on February 18, 1980. The record in-dicated that the installation of two instruments 2PT-3200A1 and 2PT-3200A2 was monitored. The installation was not com-plete and the permanent supports were not installed; QC in-spections had not been completed. During the review, it was noted that one tube end had a damaged cleanliness closure (FSK JC2-006) and several other tubings in the area had damaged closures. Additionally, temporary lighting cable was supported by a tube run. These items were brought to the attention of the instrument craft foreman who corrected them.
