C INITIAL CONDITIONS -
~0n, January 14, 1982, Radwaste was preparing to transfer filter resin beads I from the.1-B condensate phase separator to Chem Nuclear's Portable l ' Solidifying an'd Dewatering System. ;An initial clean water flush was.
performed.to confirm flow into the tank liner.
A visual inspection of the system had been performed and no difficiencies were found.
Phone contact had been established between the Radwaste Control Room and Chem Nuclear's trailer. This is maintained through the transfer During l transfer,. two people visually monitor the transfer system.
One at the i receiving tank and one watching the hoses.
EVENT DESCRIPTION
Transfer of the resin bead slurry started at 1755.
Flow into the tank , The system was shut down immediately.sAccording to gauget,ran Approximately 50 seconds into th liner was observed.
. line ruptured.
indications'in Radwaste Control Room, no more than 2 percent'or 2a0 gallons had been transferred.
This limits the spill to no more than that.
The RC window was notified at approximately.1800.
Steve Hamilton and
Betty Sutton responded and conducted the initial contro1*and cleanup operation. Resin was on the loading dock and under Chem Nuclear's .f trailer. To prevent the spread'of resin into the storm drain system, the ' south storm drain grill was immediately covered with a plastic bag and a dike constructed to prevent flow into the area of the drain. According to I Steve Hamilton, there was no visible indication that resin reached the storm drain,- only the water portion of the slurry went into the drain.
-l . A wet vacuum was used to clean up the water around the storm drain and the resin slurry at the leak area.
By 2300 most of the resin in the area had been vacuumed up and placed'in one drum. The drum read two R/hr on contact. Approximately four cubic feet of resin had been swept up.
Appropriate samples were taken to determine the extent of. the . _ contamination.
, , Hayden Caylor was called in to close the discharge from the holding pond.
The discharge was closed at 2040.
A high volume air sample was pulled.' uring and after cleanup to check for d airborne contamination.
'- , . . L Steve Hamilton had the 12-foot pi,pe tunnel closed to personnel until a radiation survey could be done.
Radiation readings in the area before cleanup were approximately 100 mR/hr at one inch over a pile of resin at the leak site and 25-35 mR/hr over'the general area.
.. Tom Gaebelein was the only person contaminated by the spill.
His shoes and pants were lightly sprayed with resin.
They were discarded, no other
contamination was found on his body.

COMMENTS ., ' The hose'line in use is steel braid reinforced with a rating of 3,000. psi.
I The steel braid was not. broken, but the area of the rupture was expanded, probablyLby. frozen water.- The rupture occurred approximately six feet from the gate valve at the dock.
, The' hose was new on November 5.'1981.
Prior to use by Chem Nuclear it was hydroed>at 225' psi for 30 minutes.
It was pressure-tested here at 145 psi on November 6, 1981.
_The pump dead head pressure is 110 psi and normal transfer' pressure is 60 . p s i'. The last transfer prior. to this incident vas January 9,1982, from 1450 to . 1802.

. In addition.to the ruptured line, the next valve 'down*s' ream from valve ' t G-16-V971 at the edge. of the leading dock had a crack, in its body.
PROBABLE CAUSE , . . With the below-freezing weather in the past week, water in the hose probably froze.
An expanded area is present at the site of the leak.
Another hose which was not'in use has been found to have a similar < expanded area. This hose is being discarded.
< l CORRECTIVE ACTIONS These will be initiated before future transfers: j kleat tracing to be used for temperatures of less than 38'F.

A heat tracing check will be part of Radwaste's operation check sheet.
~ 2.
Pretransfer flush and leak test with a dead head pressure of 140 ~ pounds will be conducted before all transfers.

, On August 11, 1982, Radwasse started to load Unit No. 1 CR vessel with resin - via the line from the "(essel to a hopper on radwaste roof northend. There is . one valve (B-246) in this line located cicua to the hopper.
~he flange was , .found to be leaking so resin loading was reopped. A trouble ticket was
written and a clearance (RW 2595) was, hung by Radwaste Opera'tions to ensure j that the d ssel das not pressurin d until the leak was fixed. The CR'essel i v w;ss then-loaded with resin using th'e AR hopper.
! - T On A' gus t 13, 1982, Radwaste mechanics removed the valve for inspection and u i repair,\\,There was no chain on the valve when they removed it.
Upon.
Inspection, the flange seal on.the pressure side (side towards CR vessel) was found to be uddxeda No veplacementgvalve w d available. The valve was flushed, reversed whichsput the better seal down towards the vessel (pressure ride)j ar4 reinstalled. LThe valve was closed and the hopper filled with demitiiralizei water. The valve was then opened permitting the water to flow ' to the CR v(sset. No lenhage was noted at that time. The trouble ticket indic~ates that,the valve was left in the open position. , ?At 01$0, August 14, 1982, p$ocessing 'o'f waste (neutralizer tank B through the "reger tskid"'(being used as a deminef 5,111tr) to a floor drain sample tank ' began. At approximately 1500, Len Eskal (contract HP) was on his way to the scaffolding warehouse when he noticef that'the hopper closest to the edge of s
the north end of radwaste roof wa's o'se'rtiowing with water. He reported this to the Radwaste Control Operator (Mrik Holland). There was not a foreman on duty for that shifi The system was secured and an Auxiliary Operator sent to inspect the hopper :h quesdon.. The Auxiliary Operator reported back to Mark Holland that the valve und w tlk hopper was chained and locked but upon , inspection he was able to raute the valve whea,1 approximately two turns towards the closed positios. Thert was nog water flow at that time.

. . , ' a-l' At approximately 1600, August 14, 1982, Jack Cook arrived at the plant.
l Cleanup of the radweste roof was in progress at that time.
Chemistr/ ] personnel were in touch with Carlton Robertson for instructions on the release ' of the storm drain basin contents to the holding pond. The basin had been l l_ pumped at 0700 with an MPC of.010.
A s~amplo taken from the basin at 1540 showed 4.763 MPC.
Repeat samples at 1730 and 2210 showeu 5.661 and 5,848 MFC respectively.
Upon instruction from Carlton Robertson, the basin was pu= ped to the holding pond at 0217 on August 15, 1982.
A composite sample drawn during release showed 4.004 MPC.
Cleanup of roof and storm drain near southend of radwaste dock carried through August 15, 1982.
A review of the tank level indicator logs showed a maximum release of approximately 8600 gallons i 50 percent to the radwaste roof.
Using the concentration from a watur sample taken north of the CR hopper on the radwaste ~ toof and the gallons above, we calculate that 1E C1 was released to the - storm drain basin.
j x gal = 3.1213 E ' uCi x 3785 ml x 8600 gal = 1.016 E " pCi x 1 ~ uCi x m m1 gal ml gal , -2 CJ = 1.016 E Ci 10 pC1-2 E&C discharge permit shows 4 E C1 Cause Description Inadequate postmaintenance testing appears to have been a contributing factor resulting in the valve (V-246) between the CR hopper and Unit No. 1 CR vessel epparently being left in a partially open condition. This permitted contaminated water to fill the hopper, flow into the radwaste roof, and irt o. the storm drains to the storm drain basir.. Corrective Action The valve has been removed and the line blind flanged.
Inspection of the valve showed it to be operating properly.
It was pressure tested at 150 psi and passed. There are three other systems similar to this CR System with the same valve.
Whenever these systems are pressurized, an Auxiliary Operator will be present on the radwaste roof during the entire pressurization cycle unless the hopper feedline has been blind flanged. This will be in effect until permanent modification of the valves is made.
Future Corrective Action: In the immediate future, a permanent change will provide for one ball valve in each hopper line in addition to the existing valve. This plant modification paperwork is being processed now.

. BSEP Engineering, assisted by E&RC, will perform a review of this system and j similar systems.
They will deter =ine if there is a need to modify them and { l.
the type of modifications necessa:y to provide continual, effective isolation.
This review, and implementation of any modifications found to be needed, might ' be an extensive project.
In the meantime, a contingency plan for handling high levels of radioactive materials that find their way into the storm drain basin should be developed.
Such a plan could include large holding tanks that could be emptied back through radwaste or via a separate cleanup system.
A current modification that will improve our ability to control the storm drain basin is the installation of a radiation in-line monitor with an automated composite sampler. The modification package will be done in November 1983. The materials for this modification should be on site by the first of December. Actual construction could possibly start in December ! - depending on the manpower constraints determined by this Fall outage. The projected construction starting time is February.
Related Concerns: . The lack of adequate numbers of foremen in radwaste has contributed to this event in direct ways such as communication and in indirect ways such as supervision and planning. To correct this situation, foremen should be identified for these positions as soon as possible.
The safety analysis for OG-08 (Mechanical Jumper Procedure C-81-9) appears inadequate in that it does not address the fact that a single valve is between radwaste material under pressure and the environment. Although it addresses the pressure factors to be applied to the system, there is no assessment of the type of valve used. The valve in question does not have an obvious closed position nor a positive indication of its closed position. This contributed to the spill.

- Initial Conditions:
On January 30, 1985, Radwaste was preparing to transfer powder resin sludge from 2B phase separator to Chem-Nuclear Systems, Inc. (CNSI), Portable Solidifying and Dewatering System. The transfer line integrity - was established through satisfactory performance of PT-45.1 at J approximately 1646. Sound powered communications was established between i ' the Radwaste Control Room Operator (RWCO) and the CNSI technician located at his equipment. This communication 'line is maintained during the transfer.and flush cycle.

i The transfer of the powder resin slurry commenced at approximately 1648.
Approximately 60 to 90 seconds into the transfer, the CNSI technician j noticed a small leak in the flexible transfer line going to the fill '] head. He immediately notified the RWC0 who shut off the transfer pump.
Approximately 30 seconds later, the automatic-flush occurred which, ruptured the line further (hole 1/4 inch in diameter). This flush has an approximate duration of 30 to 45 seconds. and is supplied from the l condensate transfer header which takes suction on Unit i or 2 condensate .I storage tank. Approximately 60 gallons water was spilled.
) ' ' Health Physics was notified immediately, the contaminated areas were , identified and placed under control.
Surveys were completed and indicated the following: no airborne contamination above background, a 4' x 45' area on the Radwaste loading dock indicated > 500,000 dpe/100 cm', a 10 x 50' area in front of the CNSI trailer' (west of Diesel Generator
Building) indicated > 200,000 dps/100 cm. A forklift in this area had the front and contaminated to the level also. The store drain in this diked area is secured by a valve (V43) under clearance RW3888. AVS was notified to begin decontamination of the contaminated areas. The major . portion of the powder spill was within the diked area.
Some powder was spilled in the area in front of the Diesel Generator Building. Standing water in the vicinity had an activity of 2E-4 yCi/cc. The area was secured and decontamination efforts commenced. The stora drain basin was sampled and no abnormal activity was detected (5.94E-7 WCi/ml).

- installation of the new process trailer.in' June 1984.
Interview with Jim Mof f ect (CNSI technician) revealed "that the flexible .. transfer line is hand-checked by.CNSI-prior to each transfer to check for visible fisws, scuff marks', abrasions, or defects of any kind.
Spare j hose is available. The hose is rated at 225 psig.
Jim.Moffatt stated j that his: inspection prior to'this transfer indicated no visible problems.

Jim Moffatt also stated that after he notified the RWC0 of the problem, he activated the closure of the solenoid operated valve located at.the head of the-flexible line, but it is a slow operating valve due to air bleed off.
Slurry flow'had been' established to-the high integrity - container prior to the line, rupture.

P,r,ybable Cause: With the severe cold weather experienced several weeks ago and the: fact ) that parts of the line lay coiled on the cement, moisture in the line - j froze ar.d weakened the hose. The hydro at 126 psig prior to the transfer j-did not cause the rupture, but as.the transfer started, the slurry flow-j aggravated;the' flaw until the rupture occurred. With the closure of the -{ -fill' head valve and because the 50V did not close:immediately, the - automatic flush put condensate transfer pressure on the: rupture and increased the size of;it'. E.

The flexible transfer line will be installed in a manner such that J it will not be in contact with the pavement and be no longer than 10 feet in length. The flexible transfer line will be hydrostatically tested at rated pressure prior to installation; rated pressure will i be at least 350 psig. CNSI will provide documentation for rating ( and test of each flexible transfer line installed.
This test will i be. performed every six months.

Until such time that the lighting described in action 6'is installed, transfer of expended resins to the mobile unit via the loading dock will take place during daylight hours, and will neither commence nor continue if in progress in the event of rain or other precipitation that could aggravate the ability to detect or prevent j ' the spread of leakage.

The CS stated that she had information that I an NES employee was changing the results of the UT examinations being conducted at the site.
WILLIAMSDN stated that the CS did not know why i the changes were being made, only that the original was changed allegedly to expedite in-service inspections.
WILLIAMSON stated that OI was initiating an investigation into the matter and requested RII assistance in possibly obtaining the records described.by the CS from the NES facility at the site.
) i Later, on 09/06/85, a meeting was conducted with P. FREDRICKSON, B.
JONES, B.
URYC, J.
VORSE and E.
WILLIAMSON, with G. MAXWELL and R.
PREVATTE on the telephone.
During this meeting it was decided that DI would go to the site themselves to obtain the records from NES as opposed to having the SRIs try to obtain the records.
WILLI AMSON advised that he would be providing additional information as it is developed.
