TUEL BUh*DLE 444 TALLING OUT OF TUEL PREP MACHINE Initial Conditions j j

l Sequence of Events At approximately 0240 on March 31, 1976, fuel bundle 444 was l

removed from the spent fuci pool storage location, using the refueling ~

bridge fuel. grapple. The bundio uns positioned over the "B" fuci prep machine and lowered into the machine. At the same time the fuel prep machine carriage was raised, the rctuei bridge operstor received " hoist  ;

loaded" light off and a "sisek cable" light on. These indications gave j the operator the opinion the fuel bundle was seated in the carrinRe. The l grapple uds opened, raised, and the bridge traversed away from the fuel  !

prep machine to a po=1Liun above the f uel pool storage racks. The refuel floor foreman again started raising the fuel prep tischine estriage upuned in order to position the fuel bundic for channel inspeettun. At this time ,

the bundio foil out of prep machine to a horizontal posi, tion across the top .

of fuel pool scornge Tschs. The refuel floor and reactor building vore evacuated and El-22 (Spent Fuel Damage) was implemented. Surveys and samples indicated no fuel damage or increased radiation. * -

, Bundle 444 vss lif ted, using slings from the jib cranes and

"C" f hook from the overbesd crane. The bundle was stored in its designated spent fuel storage location. Bundle 444 was inspected and no visible damage was detected. The bail on bundle BR-107 was bent by bundle ex-444 when it tell. Preliminary visual inspeccion of bundles BR 166 and BR-060 ,

indicated localized scratch of crud remos ! on the bail of the bundles. No indication of deformation of the bail was detected on either bundic.; These were the only bundles that were struck or could !. ave ocen struck by the falling bundle.

Inspection of the fuel prep machir.c indicated the upper roller i' guide had separated from the fuel prop carriage. The reason the guide separated from rhe fuel prep machium appears to be caused by the fuel bundle being misalist ed in the upper guide and not properly seating the i bundle in,the prep machine.

Correction Action l

1. The channelling /dechannelling proc 6 fures were revised to include two independent visual verifications of the fuci bundir noso piece being seated in the fuel prep machina. Also, prior to lifting any fuel bundle, two independent visual verifications vill be made of adequato latching of fuel bundio by grapple and prior to releasing a fuel bundle froc.

2. Refuel floor personnel were cautioned conc.ernin5 the procedure change and the necessity for visual verification of bundle saating and lif ting.

1. Tpe PNSC convened at 11:45 to review and discuss the fuel handling )

4 'j ncident which occurred this morning. J. Miller described the total i

incident from inception until the.fual bundle-in question was properly stowed. M. Jones reviewed the operator critique sheets which wera

. prepared immediately following the incidait.

The cause of this incident was the hook on the C. R. Grapple not being '

l fully engaged.

Based on the' discussion which tool. place, it was determined that the ~

following' corrective action had been successfully carried o*ut:

a. The teactor building had been evacuated,
b. Radiation surveys were completed on the refueling deck - no in-crease in activity was noted. 1
c. EI-22, as appropriate to the circumstances, had been imple'ented. m
d. All channeling activities were innediately stopped.  ;
e. The fuel bundle in question was safely'and properly restowed.

In addition, the following corrective action will be carried out prior to i using the C. R. Grapple for additional fuel handling evolutions:

a. The C. R. Grapple air valves will be labled.
b. C. R. Grapple engagement will be verified by trial lif t or by verifying Grapple engagement by visual means W/-___

. . The C. 'i. Crapple will be visually inspected and checked for proper operation at least daily when in use.

Fending inspection of the fuel bundle in question and those bundles i= mediately adjacent to it, this neeting was temporarily adjourned.

Later this af ternoon the PNSC was informed of the following inspection results:

a. A visual inspection of the fuel bundle which was inadvertently released was completed this afternoon by a CE fuel engineer. The bundle was dechanneled and both the channel and fuel were inspected.
b. There was a small dent in the channel; therefore, this channel was -

rej ected' and a new channel was installed on the bundle.

c. ,There was no visible damage to the fuel. ,

Status: This item remains open pending receipt of further information from CE.
