Mr. Martin made opening remarks and discussed the purpose of the conference.
Mr. Mendonca discussed the findings of the special inspection, involving the inoperability, due 'to miswiring, of one train of the Main Steamline Isolation Valve (MSIV) Engineered Safety Features Actuation System logic.
Specific concerns were discussed, as identified in NRC Inspection Report 50-323/85-45.
An apparent violation of NRC requirements was identified.
The meeting discussion then centered on the NRC concerns related to the event and apparent causal factors as perceived based upon the NRC's inspection activities.
The concerns addressed included the following:
Several groups within the licensee's organization missed opportunities to identify and correct the inoperability of the MSIV Train A, and similar miswiring on a feedwater isolation valv f b

The licensee's problem resolution efforts appeared to be using limited and'incomplete information.
There have been indicators of-a lack of attention to detail in licensee performance since i'ssuance of the Unit 2 license.

Further, the need for personal involvement by PGSE's top management to assure that problems are aired and resolved in'
thorough manner was discussed.
Messrs.
Maneatis and Shiffer discussed the licensee's perception of the identified problem.
They agreed that more attention to detail was needed and would be given.
Mr. Fridley presented the scenario of events related to the inoperability of the MSIV Train A actuation logic.
Mr. Shiffer discussed corrective actions taken and being taken by PGSZ to address the identified weaknesses.
This corrective action includes
'independent assessment of the licensee's problem resolution efforts by the Onsite Safety Review Group.
In response to a question from Mr. Johnson, licensee representatives stated that they had no questions regarding the NRC enforcement policy (as provided in 10 CFR 2, Appendix C).
