On December 31, 2019, the U.S. Nuclear Regulatory Commission (NRC) completed an inspection at Edwin I. Hatch Nuclear Plant. On January 23, 2020, the NRC inspectors discussed the results of this inspection with Mr. Edwin D. (Sonny) Dean and other members of your staff. The results of this inspection are documented in the enclosed report.

No findings or violations of more than minor significance were identified during this inspection.

The U.S. Nuclear Regulatory Commission (NRC) continued monitoring the licensees performance by conducting an integrated inspection at Edwin I. Hatch Nuclear Plant, in accordance with the Reactor Oversight Process. The Reactor Oversight Process is the NRCs program for overseeing the safe operation of commercial nuclear power reactors. PLANT STATUS

Unit 1 began the inspection period at rated thermal power (RTP). On November 18, 2019, the unit was down powered to 88 percent due to a loss of feedwater heating. The unit was returned to RTP on November 21, 2019. On December 2, 2019, the unit reached maximum core flow and began the end of cycle coastdown for the spring refueling outage.

Unit 2 began the inspection period at RTP. On October 10, 2019, the unit was down powered to 70 percent to repair leakage on the 7th stage feedwater heater. The unit was returned to RTP on October 11, 2019. The unit operated at or near RTP for the remainder of the inspection period.

INSPECTION SCOPES

Inspections were conducted using the appropriate portions of the inspection procedures (IPs) in effect at the beginning of the inspection unless otherwise noted. Currently approved IPs with their attached revision histories are located on the public website at http://www.nrc.gov/reading-rm/doc-collections/insp-manual/inspection-procedure/index.html. Samples were declared complete when the IP requirements most appropriate to the inspection activity were met consistent with Inspection Manual Chapter (IMC) 2515, Light-Water Reactor Inspection Program - Operations Phase. The inspectors performed plant status activities described in IMC 2515, Appendix D, Plant Status, and conducted routine reviews using IP 71152, Problem Identification and Resolution. The inspectors reviewed selected procedures and records, observed activities, and interviewed personnel to assess licensee performance and compliance with Commission rules and regulations, license conditions, site procedures, and standards.

71114.01 - Exercise Evaluation

Inspection Review (IP Section 02.01-02.11) (1 Sample)

(1) The inspectors evaluated the biennial emergency plan exercise during the week of October 21, 2019. The exercise scenario simulated a loss of the 2A recirculation pump due to excess vibrations caused by a leak in the pumps oil cooler. The excess vibrations caused the A reactor recirculation loop discharge piping to break, which led to the declaration of an Alert for loss of a fission product barrier. Due to severe thunderstorm activity in the area, multiple strong gusts of wind caused debris to fly into the switchyard, which caused the 2C startup auxiliary transformer to trip. Lightning strikes caused several other electrical malfunctions, thus creating conditions that simulated a loss of offsite power. A failure of a primary containment penetration seal occurred and caused drywell pressure to decrease rapidly, thus creating conditions that met the threshold for declaring a Site Area Emergency. Lastly, due to high gas gap pressures, several older fuel bundles released radioactive gas into the drywell. This caused radiation levels to increase rapidly and simulated conditions that met the threshold for a General Emergency and allowed the Offsite Response Organizations to demonstrate their ability to implement emergency actions.

The inspectors evaluated the following licensee event reports (LERs):

(1) LER 05000366/2019-004-00, High Pressure Coolant Injection System (HPCI)

Inoperable Due To Brief Loss of Power to HPCI Pump Flow Controller (ADAMS accession: ML19289E222). The inspectors determined that it was not reasonable to foresee or correct the cause discussed in the LER therefore no performance deficiency was identified. The inspectors also concluded that no violation of NRC requirements occurred.

INSPECTION RESULTS

Observation: Semi-Annual Trend Review Identified Adverse Trend in the Intake Components Residual Heat Removal Service Water and Plant Service Water Health.

71152 The inspectors performed a trend analysis on the licensees CAP to identify trends that could indicate the existence of a more significant safety issue. The inspectors focused their review on human performance trends, but also considered the results of inspector daily condition report screenings, licensee trending efforts, and licensee human performance results. The review nominally considered the 6-month period of May 2019 through November 2019, although some examples extended beyond those dates when the scope of the trend warranted. The inspectors compared their results with the licensees analysis of trends.

Additionally, the inspectors reviewed the adequacy of corrective actions associated with a sample of the issues identified in the licensees condition reports.

The Hatch Service Water Systems on both units have experienced water quality issues due to the increased concentrations of sand and silt due to low river level and high sandbar conditions in the Altamaha river at the intake structure. Two contributors to this problem were; 1) The dredging of the river and cleaning of the intake suction pit were performed later than normal this year. 2) The dredging of the river last year removed minimal material (13,575 cubic yards vice the average annual removal of 45,000 cubic yards). The water quality issues have resulted in an increase in strainer backwashes and degradation of several service water pumps as documented in the following condition reports (CR):

The licensee initiated CR 10670010, Comprehensive Causal Product Needed - Intake Component Health, to address the adverse trend in service water equipment reliability. The inspectors reviewed the CR and determined that the licensee was adequately addressing the adverse trend. The inspectors will continue to monitor the licensees actions to address this negative trend.
