Nos. 50-321/78-35 and 50-366/78-43 -2- Contacts Fith Local Officials The inspector contacted local officials of Appling County and the City of Baxley, Georgia to introduce himself and explain his role at the plant. Personnel contacted also included those in city and county law enforcement agencie In addition, the inspector discussed NRC objectives with the Executive Director of the Altamaha Area Planning and Development Commissio This commission roordinates regional development plans in a five county areas surrounding Plant Hatch. A Public meeting with officials is planned for November 16, 197 . Contacts With Local Media The inspector met with the Editor of the Baxley News-Banner, the local weekly, to explain the revised inspection program. The paper ran an article on September 7, 1978, which summarized the interview. The regional Public Af f airs Of ficer accompanied the resident inspector on September 20, 1978 to visit media contacts in the surrounding communitie Radio station managers and newspaper editors in Vidalit an.1 Lyons were given briefings on the revised inspection program. The radio station manager in Baxley was also contacte > Test Program Review and Evaluation (Unit 2) The inspector reviewed the test sequencing document, RNP-2-10000 series, to determine the status of start-up testing on Unit 2 on a daily basis. The startup test engineers log, control room log, and shift supervisors log were also reviewed daily. The inspector observed control room operations to ascertain that operator actions were in conformance with regulatory requirements, technical specifications, and administrative procedure . Plant Tours Plant tours were periodically conducted during the inspection interval to verify that monitoring equipment was recording as required, equipment was properly tagged, operations personnel were aware of plant conditions, and that plant housekeeping efforts were adequate. The inspector also toured the drywell and torus on Unit 2. In the torus on Unit 2, the inspector noted that a junction box panel cover was missing. The licensee replaced the missing cover prior to closcout of the toru . 50-366/1978-05 RCIC Turbine Tripped During Test 50-366/1978-06 Average Drywell Temperature Greater Than 135 F 50-366/1978-07 Modified Condenser Low Vacuum Trip Signal 50-366/1978-08 Drywell ECCS Initiating Switch Set High 50-366/1978-11 Hanger Lugs on Feedwater Line Not Inspected 50-366/1978-15 Torus Vacumn Relief Valve Inoperable 50-366/1978-06 involved drywell temperatures which exceeded the 135 F limit specified in Tecnnical Specification 3.6. The lisensee installed additional permanent cooling ducting and this appears to have corrected the problem. A supplemental report to this LER remains to be submitte . Access Training Program The inspector participated in the training program conducted by the licensee in order to gain unrestricted access to the site area. The training took approximately ten hours and covered radiation protection practices, emergency planning, and security procedures. The inspector noted that the material content was presented as required by HNP-203, General Employee Training. The inspector had no further question . Airlock Door Malfunction On September 23, 1978, following an inspection in the drywell at low power, three men were unable to leave the drywell due to a door interlock malfunction. The men were able to exit through the control rod drive removal hatch approximately 30 minutes later. One individual who had a history of high blood pressure was sent to the hospital for a checkup and was released. No contamination or overexposures were noted. The licensee is taking the steps necessary to modify the door interlocking mechanism to prevent recurrenc . Power Ascension Test Witnessing (Unit 2) The inspector witnessed portions of the turbine startup testing . conducted as at part of HNP-2-10300, the power ascension sequencing document. The inspector observed over-all crew performance, adherance to applicable technical specifications, coordination of activities between the on-shift crew and vendor organizations, satisfaction of initial conditions, and ability to meet applicable acceptance criteri The generator was paralleled with the utility grid for the first time on September 22, 1978, and supplied 10 megawatts of electric power for about 10 minute I

Nos. 50-321/78-35 and 50-366/78-43 -4-1 Diesel Generator Start Logic Error On September 20, 1978, a problem was discovered in the diesel generator logi The installed logic could result in a condition in which an engine could not be restarted from the control room under emergency conditions of a LOCA or loss of offsite power (LOSP) af ter either a failure to start initially or a protective shutdown while runnin The problem occurs when the operator resets the diesel engine in the control room. As soon as the reset is initialized, air is supplied to the engine for 7 seconds to roll it. However, fuel is inhibited f rom entering the engine for 100 seconds under the present logic resulting in f ailure to star As long as the LOCA or LOSP signal is present, the engine will not start after the first failure. The engine could be started at the local panel. The licensee reported this problem as a 14 day LER and is processing a design change to rectify the circui . Personnel Cha_nge As of September 22, 1978, Mr. R. 'I . Nix replaced Mr. M. O. Schliessman as Superintendent of Maintenance. The inspector verified that Mr. Nix met the requirements of Section 4.2.3 of ANSI N18.1-1971 concerning power plant and nuclear experienc . Plant Review Board (PRB) Minutes Review The inspector reviewed the minutes of the Plant Review Board for the period f rom July 4,1978 till September 15, 1978. Meeting periodicity, quorum requirements, and the content of the minutes were evaluated against the requirements of Section 6 of facility technical specifica-tion The inspector noted two instances where weekend meetings were held and the quorum requirements was met by calling a fourth PRB member by phone The inspector discussed these instances with the licensee and had no f urther question . Limiting Conditions for Operational (Unit 2) During the reporting period, the inspector verified compliance with various limiting conditions for operation (LCOs). This verification was accomplished by direct observation of monitoring instrumenta-tional, valve positions, switch positions, or review of logs and records. The licensees compliance with selected LCO action statements was reviewed on selected occurrances, The inspector will have inspected licensee compliance with each LCO at least once by the completion of the power ascension test progra j
