SUMMARY

The U.S. Nuclear Regulatory Commission (NRC) continued monitoring the licensees performance by conducting an integrated inspection at Donald C. Cook Nuclear Plant, in accordance with the Reactor Oversight Process. The Reactor Oversight Process is the NRCs program for overseeing the safe operation of commercial nuclear power reactors. PLANT STATUS

Unit 1 began the inspection period at rated thermal power. On July 27, 2021, Unit 1 was shut down to Mode 3 to repair a circulating water discharge valve issue and was returned to full power on August 2, 2021, where it remained through the end of the inspection quarter. Unit 2 was shut down at the beginning of the inspection period due to a steam leak related to the main turbine. After repairs, Unit 2 returned to full power on July 10, 2021, where it remained through the end of the inspection quarter.

INSPECTION SCOPES

Inspections were conducted using the appropriate portions of the inspection procedures (IPs) in effect at the beginning of the inspection unless otherwise noted. Currently approved IPs with their attached revision histories are located on the public website at http://www.nrc.gov/reading-rm/doc-collections/insp-manual/inspection-procedure/index.html. Samples were declared complete when the IP requirements most appropriate to the inspection activity were met consistent with Inspection Manual Chapter (IMC) 2515, Light-Water Reactor Inspection Program - Operations Phase. The inspectors reviewed selected procedures and records, observed activities, and interviewed personnel to assess licensee performance and compliance with Commission rules and regulations, license conditions, site procedures, and standards.

Starting on March 20, 2020, in response to the National Emergency declared by the President of the United States on the public health risks of the coronavirus (COVID-19), resident and regional inspectors were directed to begin telework and to remotely access licensee information using available technology. During this time, the resident inspectors performed periodic site visits each week, increasing the amount of time on site as local COVID-19 conditions permitted.

As part of their onsite activities, resident inspectors conducted plant status activities as described in IMC 2515, Appendix D; observed risk significant activities; and completed on site portions of IPs. In addition, resident and regional baseline inspections were evaluated to determine if all or a portion of the objectives and requirements stated in the IP could be performed remotely. If the inspections could be performed remotely, they were conducted per the applicable IP. In some cases, portions of an IP were completed remotely and on site. The inspections documented below met the objectives and requirements for completion of the IP.

71111.01 - Adverse Weather Protection

OTHER ACTIVITIES

- TEMPORARY INSTRUCTIONS, INFREQUENT AND ABNORMAL 60855 - Operation of an ISFSI The inspectors evaluated the licensees Independent Spent Fuel Storage Installation (ISFSI)cask loading activities on July 26 through July 30, 2021. Specifically, the inspectors observed the following activities during the loading of cask 47:

Heavy lifts of the loaded transfer cask from the spent fuel pool to the cask processing area Closure welding and non-destructive evaluations Canister processing including forced helium dehydration and backfill with helium Stack-up and download of the Multi-Purpose Canister (MPC) from the HI-TRAC into the storage cask (HI-STORM)

Radiological field surveys The inspectors performed walkdowns of the ISFSI pad, including performing independent radiation surveys, and walkdowns of the ISFSI haul path and vertical cask transporter.

The inspectors evaluated the following:

Spent fuel selected for loading into dry cask storage during this loading campaign Selected corrective action program action requests Selected 72.48 screenings and evaluations

INSPECTION RESULTS

Observation: Unit 1 Circulating Water Flow Was Degraded Requiring a Power 71153 Reduction The inspectors responded to, and monitored, licensee actions for an unexpected plant condition involving the Unit 1 Circulating Water Pump. At approximately 0500 on July 21, 2021 the Unit 1 Control Room heard a loud boom and saw a

.8 lowering of vacuum

in the Unit 1 Main Condenser. Immediate operational actions stabilized vacuum with no other immediate risk significant plant impacts. Operations prepared for, and executed, an approximate 10 percent down power on Unit 1 while dispatched Operations and Maintenance personnel performed field investigation. Inspectors observed licensee implementation of its Failure Investigation Process (FIP) for further investigation of cause and resolution of the issue. The FIP team concluded the cause was related to a failure of the #13 Circulating Water Pump Discharge Valve, 1-WMO-13. Inspectors reviewed the resulting short and longer-term actions. Specifically, the licensee developed an ODMI for monitoring Circulating Water Pump #13 parameters, developed a temporary change to install a pressure gauge to monitor #13 Circulating Water Pump Discharge pressure, and planned for the repair/replacement of the discharge valve as part of a forced outage. The licensee documented the event within its corrective action program via AR 2021-6321, Circulating Water Flow is Degraded and Required a Power Reduction. Additionally, the inspectors determined immediate licensee actions were reasonable given the overall safety significance of the issue. Based on the inspector's review, no concerns were identified with the immediate licensee actions. Since resolution of the issue required a forced outage, inspectors monitored those activities through Inspection Procedure 71111.20, Refueling and Other Outage Activities.
