An Enforcement Conference was conducted in the NRC Region I Office to discuss the l circumstances surrounding the discovery of nine shut root isolation valves for the Steam Generator Pressure Transmitters which rendered pressure Channels B, C, and D and their associated protective functions inoperable. Also discussed was the i

discovery of the inoperability of the Channel A Steam Generator Low Pressure trip I function for the Reactor Protective System due to an inadequate design modification.

l The root cause of the events and the corrective action to prevent recurrence were outlined by the licensee. The corrective actions included an in-depth plan for

control of all root valves as well as a review of past plant modifications and a l functional test program to evaluate proper installation of those modifications.
