PRE DECISIONAL COOPER NUCLEAR STATION EXECUTIVE SUMMARY SEMIANNUAL PLANT PERFORMANCE REVIEW 94 04 October 12, 1994 SALP CYCLE 012 (April 25, 1993. Thru January 21, 1995)
I.
01ERVIE.W The licensee's overall performance continued to be marginally adequate.
In all functional areas. performance weaknesses were identified; however, significant concerns were identified in the areas of maintenance, surveillance, and engineering as a result of the identification of the inoperability of safety systems, lhe licensee has shown some improvement in the review of operational experience information in that operational-type is s are t)eing identified.
This licensee 3erformance level was reached only aft, continuous cversight of this area has Jeen focused on by the NRC.
The licensee imalemented changes in key management positions (Site Manager.
Manager of Oversight, Licensing Manager. 0A Manager. Plant Manager and Engineering Manager) at the facility: however. limited opportunities have been available to assess the overall performance of these individuals.
As a result of an investigation performed by 01. it was identified that the performance of the Station Operations Review Committee was severely lacking in implementing its responsibilities for ensuring safe operation of the facility.
The independent oversight activities performed by 0A have been marginally adequate as noted in OA's review of Problem Resolution Team performance.
A diagnostic self-assessment was performed by industry, non-licensee personnel.
The results of the diagnostic indicated that management was not providing the appropriate level of oversight of the operation of the facility, it was noted, during review of an abnormal scram, that the licensee was focused on restart of the ]lant rather than identification of the cause of the scram and correction of t7e anomalies.
II.
SUMMARY OF SIGNIFICANT REGULATORY ISSUES An enforcement conference was conducted on April 4, 1994, to discuss issues raised during the Operational Safety Team inspection conducted during November 1993.
A management meeting was also held immediately following the enforcement conference.
As a result of the conference, eight Severity Level IV violations were issued without a civil penalty.
On September 16. 1994, an enforcement conference was held to discuss the inoperability of both diesel generators, primary containment, and the control room envelope.
An enforcement decision has not'yet been made regarding these issues: however, a recommendation has been made, and if accepted, the enforcemeni. package will require Commission approval prior to issuance.

WEAKNESSES:
Shutdown cooling was lost three senuential times before it was l
decided to determine the cause of the losses.
Communications between onshift personnel needs improvement.
Contrary to a senior management directive, forced core cooling was suspenc"d for hours longer than had originally been agreed to.
An operator error was committed when the control board switch for the wrong valve was operated during a surveillance test.
As a result, d
independent verification is now required for all operator actions, while the
action is being performed.
The operators were noted to be in a relaxed-posture during a backshift tour by the residcat inspector.
Numerous valve checklist discrepancies were identified that were caused by design changes, which added or removed valves from systems. not being incorporated into valve checklists.
I PERFORMANCE ASSESSMENT WITH RECOMMENDATIONS:
Overall performance in this area was mixed.
Although rcspc ;e to events was considered good, weaknesses were noted in the performance of the operations staff.
Recommended MIP changes include:
(1) Close Modules 40500, when the inspection report is issued, by reference as these core and regional initiative modules will be completed by the NRC's Special Evaluation Team (SET). (2) increase the hours for monthly resident observation of operations to provide hours for increased inspection activity by the additional individuals supplementing the resident staff. and (3) add hours for a new occurrence of Module 71707 to allow for observation of the performance of operating crews on the simulator.

STRENGTHS:
Generally good troubleshooting activities once equipment problems were identified.
WEAKNESSES:
Deficiencies in the maintenance and testing of safety-related systems were noted and resulted in systems being found inoperable.
The systems include both diesel generators control room envelo)c. and primary containment.
As a result of the review of the concerns wit 1 the operability of equipment and systems, it was noted that the licensee used preconditioning techniques to ensure that equipment and systems would pass the required surveillance tests.
Because of ineffective planning and scheduling, equipment remained out of service for extended periods.
Repetitive maintenance on equipment was performed because maintenance personnel were not correcting the problem the first time.
The licensee has experienced problems with foreign material exclusion, which has caused the failure of motor-operated valves to function properly.
Over reliance on the akill of the craft has resulted in less than adequate maintenance procedure; being provided for use.
PERFORMANCE ASSESSMENT WITH RECOMMENDATION:..
The licensee continues to experience significant difficulties in the implementation of the maintenance and surveillance programs.
The SET is currently performing a review of these programs.
Based on this review, and the performance of the routine inspection program, additional regional initiatives will be recommended in the near future.
Recommended MIP l
changes include deletion of Modules 49001, 703/0. and 73753.

Rereview of tne information has identified equipment and system operational concerns.
The actions specified on condition reports were narrowly focused.

PERFORMANCE ASSESSMENT WITH RECOMMENDATIONS:
The engineering organization continued to display a weak performance.
Recommended MlP changes include:
(1) deletion of Module 37550, as this core module will be completed by the SET (to be deleted after inspection report issued). and (2) additional hours for Module 92903 to allow for additional followup activities in the area of engineering open items.

STRENGTHS:
Inspections identified good porformance in the areas of ALARA, environmental monitoring, and radiological waste programs.
Good self-assessments were noted in these programs.
Generally good management and implementation of the security program was also noted.
WEAKNESSES: Weaknesses (corrective measures for equipment, package searches, and access authorization) were noted in the implementation of the security program.
Weaknesses with the implementation of the fire protection program wcre noted, which included Appendix A of BIP 9.5 1 fire barriers Appendix R exemptions, and the installation of fire doors.
PERFORMANCE ASSESSMENT WITH RECOMMENDATIONS: Overall performance in this area was good.
Recommended MlP changes are to add Modules 82301 (core). 82301 (regional initiative), and 82302 as there are two emergency exercises during this SALP cycle.
