,c COOPER NUCLEAR STATION EXECUTIVE SUMMARY SEMIANNUAL PLANT PERFORMANCE REVIEW 94 02-April 19, 1994 SALP CYCLE 012 (APRIL 25, 1993, THRU October 22, 1994)

I.

OVERVIEW The licensee's overall performance continued to be marginal in several areas, with a slight decrease noted in overall performance in maintenance and engineering. A lack of a questioning attitude by plant personnel continued to challenge the organization.

Significant weaknesses continued in the identification and resolution of problems.

Weak corrective actions and root cause analysis continued to degrade performance Upper management organizational changes returned managers to the plant site; a policy instigated to improve the Cooper culture.

These organizational changes were recently implemented and have not yet fostered a significant change.

II.

SUMMARY OF SIGNIFICANT REGULATORY ISSUES An enforcement conference was conducted on January 31, 1994, which addressed concerns associated with the loss of both emergency diesel generators.

Two Severity Level IV violations were identified involving failure to follow procedures, procedures that were inappropriate to the circumstances, and inadequate corrective actions.

No Civil Penalty was proposed.

An enforcement. conference was conducted on April 4, 1994, to discuss issues raised during the operational safety team inspection conducted at Coo)er Nuclear Station during November 1993.

A management meeting was also 1 eld immediately following the enforcement conference.

STRENGTHS: Good communications by operations personnel in a recent'EP exercise and improved overall performance in stressful simulator scenarios was observed. Recent scrams challenged the operators, whose performance was noted-as good. Operations and training department interface had improved.

Management had not established operator expedations for routine shif t activities, and may have contributed to a centWued l ack of questioning attitude by the operators during annunciator regoase ard operability determinations.

PERFORMANCE ASSESSMENT WITH RECOMMENDATIONS:

The licensee's )erformance in this area remained mixed and indicated a continued high threslold for identifying problems.

The day-to-day evolutions were handled well as was the challenges of recent reactor scrams.

However, the contmued lack of a questioning attitude by the operators and the failure of licensee management to promptly address plant and personnel issues continued to result in unnecessary challenges to plant operation.

None identified WEAKNESSES:

Continued reliance on skill-of-craft produced increasing numbers of problems where procedures were inadequate.

Maintenance work process expectations were not well defined have not been followed as intended, and management had failed to conduct observation audits.

This resulted in numerous testing and maintenance weaknesses in various safety-related systems and ineffective problem resolution.

PERFORMANCE ASSESSMENT WITH RECOMMENDATIONS:

The performance in this area remained consistent with the previous performance assessment.

Work-arounds and an inadequate infrastructure to support maintenance activities were not aggressively pursued by mannement, and weaknesses in this area are expected to continue.

STRENGTHS:

Management initiated increased engineering involvement in problem resolution and more clearly outlined the system engineers' responsibilities and increased his perception of system ownership.

Additionally, a fairly large increase to the engineering staff is ongoing.

These initiatives are fairly new and their effectiveness is not yet apparent.

WEAKNESSES:

Although improved over this period, significant weaknesses still exist in problem resolution.

System engineers are weak in system knowledge, slow to respond to industry issues, and also do not aggressively pursue concerns.

The licensee's implementation of the modification and temporary modification processes was weak in several instances.

PERFORMANCE ASSESSMENT WITH RECOMMENDATIONS:

t>erformance in this area has continued to decline.

Initiatives recently put in place to increase the engineering staff and focus expectations of the system engineer may result in improvement. but these initiatives are not yet fully implemented and their effectiveness is not yet apparent.

STRENGTHS:

Continued and improving strong radiological controls program for ALARA and exposure control.

The security organization was effectively managed.

WEAKNESSES:

Several miscellaneous training programs showed weaknesses, most notably in fire brigade training fire door inspections, and Shift Technical Advisor training.

PERFORMANCE ASSESSMENT WITH RECOMMENDATIONS:

Performance in this area was generally good with mixed performance in emergency preparedness and significant weaknesses in fire brigade and STA training program implementation.
