COOPER NUCLEAR STATION EXECUTIVE SUMMARY QUARTERLY PLANT PERFORMANCE REVIEW 95 01 January 31. 1995 SALP CYCLE 012 (APRIL 25, 1993, THROUGH JULY 8, 1995)

I.

DVERVIEW The plant was shut down May 25. 1994, because of the inoperability of the emergency diesel ManualChapter03$eneratorsandhasremainedshutdownunderCAlsandNRC 0 due to identified safety and regulatory issues.

During this assessment period, a new licensee management team was put in position and significant licensees' resources were dedicated toward the implementation of the licensee's restart readiness program.

The restart readiness program consisted of those items that the NRC staff and the licensee agreed were required to be corrected prior to restart of the facility.

NRC inspection resources have been focused on evaluating licensee actions with respect to its restart readiness program.

As a result of these inspections.

. appeared that the licensee's performance was good in that improvements were ated in the areas reviewed.

PERFORMANCE ASSESSMENT WITH RECOMMENDATIONS:

Performance in this area was mixed.

The operation of the facility during routine day-to-day activities were performed satisfactory: however, some dif ficulties were encountered by the operations staff.

The staff demonstrated problems with their ability to follow procedures and, as a result, condition tagging errors and systen. operational errors (e.g., draining of boric acid into the primary system from the SLC system) were made.

Root causes for these problems included items such as inadequate prejob briefings, poor conmunications between the control room operator and the operator in the field, and failure to follow procedures as written.

Due to the extended shutdown, around the clock coverage for power ascension will be provided.

The ficensee's current plan is for restart the week of February 5.

To ensure that adequate hours are available for this inspection activity, additional hours will be added to the MIP.

PERFORMANCE ASSESSMENT WITH RECOMMENDATIONS:

During this assessment period, it appears that the maintenance staff has perforned well, with some exceptions.

Due to poor prejob briefings a technician inadvertently dropped a source range monitor onto the floor because he disassembled the monitor in an inappropriate manner.

In another observation, operators failed to document in an official test record substantial indicated flow oscillations identified during a core spray surveillance. Additional observations by supalemental resident inspectors will be performed during power ascension as tle performance of maintenance and surveillance activities was a root cause for the extended plant shutdown.

The performance of the engineering organization has been good over this rating period.

The engineering resources have been severely taxed over the last few months as a result of the need to resolve the multitude of issues that require resolution prior to startup. Additional hours will be added to the MlP to allow for followup of the numerous issues being ressived by engineering.

PERFORMANCE ASSESSMENT WITH RECOMMENDATIONS:

A number of issues within the fire protection program have been identified by the licensee during a review of the program.

These issues are currently being resolved.

In addition, station emergency lighting was found to be deficient.

The design basis for the lititing cannot be met at some locations in the plant where identified no lighting existed and operator actions were required._

The implementation of the HP and security programs appeared to be effective.

The licensee successfully demonstrated its emergency preparedness capability in December.
