Engineering Support Section 2 Inspection Summary Inspection on June 11 and 13, 1979 (Report No. 50-295/79-12)

Areas Inspected: Review of radiographs for the feedwater nozzles for the four Unit 1 steam generators.

Also, a meeting was held with the licensee on June 13, 1979, to further review Unit 1 steam generator feedwater nozzles radiographs and discuss ultrasonic test data.

The inspection involved a total of 8 inspector-hours onsite and et the Maywood office by two NRC inspectors.

Results: No items of notrompliance or deviations were identified.

Functional or Program Areas Inspected Following the unit trip on June 8, 1979, (Unit 1) the licensee decided to take the unit to a cold shutdown in order to perform volumetric inspection of all four steam generator feedwater nozzles to feedwater pipe welds.

These inspections were initiated as a result of the NRR Letter (Stello)

to all Pressurized Water Reactor Licensees, dated May 25, 1979, info rming the licensees of the cracks in feedwater lines at the D. C. Cook Plant.

Volumetric inspections conducted consisted of radiographic and ultrasonic testing methods.

Initial radiographic testing results revealed what was believed to be questionable indicators in all four steam generator nozzle welds. Further radiographic '.asting and additional testing by ultrasonic testing revealed no rejectab indications, including cracking. The radiographic testing resultr of June 10 and 11, 1979, were also compared with the original constre ton radiographs that were taken in 1971 and 1972 with no noticeabi enanges in the integrity of welds.

On June 11, 1979, the inspector went to the Zion Plant to review the radiographs, recent as well as the original construction radiographs.

The inspection results were that no rejectable indications were present in the weld areas and no noticeable changes had taken place between the original and recent radiographs. Ultrasonic inspection was further utilized to comfirm weld quality.

The radiographs did, however, reveal a questionable area, a dark line (sometimes subtle, other times distinct) that appeared to be in the base materials, both nozzle and pipe / fitting sides.

The lines appear to be the result of radiographic density changes resulting from a change in material section thickness.

These areas also appear to be located in the weld heat affected zone.

On June 13, 1979, NRC personnel met with licensee personnel at the CECO offices in Maywood, Illinois, to discuss the volumetric examination results and to compare the Zion radiographs to the D. C. Cook radiographs,

'that did show cracking.

This meeting concluded that no rejectable indi-cations had been detected in the feedwater nozzle weld areas. The ques-tionable areas, noted above (dark line), that did appear in the radiographs were discussed in detail with the licensee.

As a result of these discussions to determine what these areas actually are, the licens a committed to perform the following:

An in depth review of existing records both at the Zion site and
