SUMMARY: The licensed operator requalification training program was rated as satisfactory. Requr.lification written examinations and operating tests were administered to

'

seven senior reactor operators (SROs) and eight reactor operators (ROs). These examinations were administered in accordance with Revision 7 to NUREG-1021. All fifteen operators passed their examinations, as graded concurrently and independently by the NRC

and GPU. These same fifteen licensed operators also comprised three operating crews with five licensed operators being assigned per crew. All three crews were evaluated as satisfactory by both the NRC and GPU during the conduct of the simulator scenarios.

All three portions of the examination were found to be technically challenging and well administered. Upper management's involvement with evaluating crew performance during the simulator portion of the examination was a programmatic strength.

2.0 PROGRAM EVALUATION RESULTS:

2.1 Background

On December 3,1992, General Public Utilities (GPU) representatives from the Three Mile Island, Unit 1 (TM1-1) operations and training departments met in Region I with the NRC to discuss the scheduled February 1993 requalification examination. The purpose of the meeting was to discuss various aspects of proposed Revision 7 to the Examiner Standards, under which the requalification examination would be conducted. Specifically, the propsed Revision 7, which was issued for comment in July 1992, was used for the examination. A slightly revised version of Revision 7 was approved in January 1993, subsequent to the meeting, but was not used for this exam. The majority of discussion dealt with major differences between the Revision 6 and Revision 7. Further discussion dealt with administrative aspects of the examination, such as scheduling, crew composition and rotation, etc.

From January 19 - 21, 1993, the NRC reviewed the proposed examination with training and operatians representatives at the TMI-1 site. This detailed review included a review of the adequacy of the written examination questions, a walkdown of all job performance measures (JPMs) and the validation of the simulator scenarios. i critical tasks. Also, the NRC met with the majority of those licensed operators to be examined in February in order to explain the examination process and answer any questions that they may have had.

2.2 Overall Rating: Satisfactory The Three Mile Island, Unit 1 program for licensed operator requalification training was rated satisfactory in accordance with the criteria established in the proposed Revision 7 of ES-601, " Administration of NRC Requalification Program Evalration." Those criteria are as follows:

A.

At least 75% of all operators pass all portions of the examination. This number includes operators who participate in the simulator examination for the purpose of meeting crew composition requirements.

NRC grading is the only consideration for this criterion. There were no individuals who participated in the simulator portion of the examination in order to meet crew requirements. Fifteen of fifteen operators (100%) passed the examination.

B.

At least two-thirds (66%) of the crews pass the simulator examination.

The requalification program met these criteria.

2.3 Programmatic Strengths and Weaknesses A.

Strengths:

The examination was well scheduled and as a result, very few delays were encountered by the operators during the examination process.

e TMl-1 training evaluators were ver,y mowledgeable in all aspects of the exam process and presented themselves in a very professional manner during the administration of the exam. The evaluation team included a management representative (plant manager or site vice president), who reviewed the overall crew performance and communication. The examiners assessed this approach to be an example of exec 11ent management involvement.

The training department interfaced effectively with the operations department.

Examination security was excellent during all aspects of the examination.

e The training department has created various video tapes for training on e

simulated in-plant JPMs. These JPM videos depict the manual operation of a valve or component which is required during emergency conditions. The examiners reviewed two of these tapes, which included JPMs that were part of the examination, and found them to be well structured and very informative.

B.

Weaknesses:

On a few occasions, GPU evaluators, when questioning the operators during

the operational portion of the examination, provided feedback regarding the correctness of an enswer. The NRC examiners subsequently reminded the evaluators that this was inappropriate during the conduct of an examination.

3.0 EXIT MEETING On February 5,1993, at the conclusion of the requalification examination, the NRC conducted an exit meeting at the Three Mile Island training facility. Those personnel in attendance are noted below. Examination development and conduct, general observations noted during the exam, and programmatic strengths and weaknesses were discussed.

Examination results, as determined by both the NRC and GPU, were also discussed.

Additionally, the NRC made the following comments regarding general observations noted during the examination process.

Plant housekeeping improvements were evidenced by the ongoing painting of walls e

and Doors throughout the plant. General condition of the overall plant was good.

Excellent working facilities were provided to the NRC examination team, including a e

locked filing cabinet within a locked office area.

e Reference material provided to the NRC was excellent. All material was indexed and appropriately tabbed.

l On two occasions, the simulator exhibited problems which resulted in examination

delays while the problem was investigated. On both occasions, the problem was quickly identified and corrected. This quick turnaround time helped to avoid excessive examination delays which can ultimately contribute to operator (examinee)

stress.
