Operating License No. DPR-50 Docket No. 50-289 Facility Examination Report to the NRC This report provides the facility evaluation of the NRC-administered licensed operator requalification examinations administered to 15 individuals at THI-1 during the period of February 1 - 4, 1993. This report is being prepared at the request of the NRC Chief Examiner, P. H. Bissett.

Overall, THI-1 has rated its licensed operator requalification program as satisfactory. The NRC and THI administered examinations resulted in 15 of the 15 examinees evaluated as having passed both the written examination and the operating test.

The following attachments detail the various aspects of the examinations:

O ATTACIIMENT 2 SIMULATION FACILITY REPORT Facility Licensee: TMI-l Facility Docke.t No.: 50-289 Requalification Examinations Administered from: February 1 - 5, 1993 This form is used only to report observations. These observations do not constitute audit or inspection findings and are not, without further verification and review, indicative of non-compliance with 10 CFR 55.45(b). These observations do not affect NRC certification or approval of the simulation facility other than to provide information which may be used in future evaluations. No licensee action is required in response to these observations.

During the conduct of the simulator portion of the operating tests, the following items were observed.

On two occasions - one being the simulator examination and the other being the performance of JPMs, various system components began to function erroneously for the given plant conditions. Fortunately, none of the malfunctions factored into the performance of the scenario or JPM. A hardware " chip" problem was identified on both occasions as the contributor to the malfunctions. The TMI-1 simulator technicians quickly identified and corrected the problem on both occasions.
