Purpose of the Inspection The inspector stated that the purpose of this inspection was to audit the documentation associated with nondestructive exam-ination of welds performed during the 1976 refueling outage, as a part of the ISI program, and to review the construction records identified in the licensee's letter of November 3, 1975 to the NRC Region I Office which concerned misplaced preopera-tional inspection data for certain welds.

program, and to review construction records identified in the licensea's letter of November 3, 1975 to the NRC Region I Office which concerned misplaced preoperational inspection data for certain welds.

ASME Section XI, Inservice Insoection Proeram The inspector audited the documentation associated with th.

implementation of the ISI program described in Section 4.2 of the Facility Technical Specifications.

Table 4.2-1 of this Specification 1:entifies exa=ination categories, areas to be examined and stipulates methods, extent and frequency of examinations.

i The inspector also audited preliminary results of the examinations performed on the welds identified as SP-069, MU-167, MU-073,

--j SG-006-S, SG-013-S and SR-010-BM for co=pliance with procedural requirements.

l The above audits by the inspector evidenced that the ISI program is being implemented in accordance with, Technical Specification requirements.

The inspector also witnessed the calibration of ultrasonic equipment for the volumetric examination of the reactor pressure vessel-to-flange weld and the flange ligament areas. The inspector i

observed thac equipment calibration requirements stipulated in Procedure N1P-004 were being followed and that the personnel involved with this calibration were qualified in accordance with procedural requirements.

Eddy Current Testine Procedure, Steam Generator Tubes The licensee issued work request No. 1233 dated 10/30/75 specifying that any eddy current examination (ET) be performed on 3 percent of the tubes in each steam generator.

The examina-tion contractor developed Procedure No. 41EC-011, revision 0, i

dated 12/15/75 for ET of the steam generator tubes.

This procedure was developed to satisfy the requirements of Regalatory Guide 1.83.

-6-The inspector reviewed the above procedure and had no questions concerning the procedure.

Reestablishment of Preonerational Inspection Data On November 3, 1975, the licensee notified the NRC Region I Office by letter that preoperational inspection data of certain welds had been misplaced and that a part of this data would be reestablished during the ensuing ISI program. Additionally, the letter identified areas / components with documentation trace-i able to examinations performed during construction which would j

be used in lieu of the preoperational inspection data.

The inspector audited the ISI schedule and found that the

identified items for reexamination were included in the 1976 refueling outage schedule.

The faspector also audited reports

of those nondestructive examinations which were performed during construction and verified that the data was available as stated in the licensee's letter of Nove=ber 3, 1975.

The inspector reviewed the material certifications for the

Steam Generator and Pressurizer manway cover studs and nuts.

examined as required.

l The inspector reviewed the Pre-Examination Final Report (PEFR)

which is a compilation of all preoperational inspection data.

The PEFR provided documentary evidence that the nondestructive examinations had been perfor=ed and that the results were as reported in the licensee's letter of Nove=ber 3, 1975.

NDE Test Personnel Oualification Records The inspector audited the qualification records of the examina-tion contractor's personnel involved with the 1976 ISI examinations.

This review included records such as for=al training, job experience, results of qualifying examinations and physical examination reports. The documents indicated that the require-ments of SNT-TC-1A (governing standard) had been met, except that visual examination reports for certain temporary employees did not clearly delineate all visual limitations.

This ites is unresolved pending a subsequent review of the modified eye examination reports.
