The inspector asked what precedure the licenceo ** uld follev in

reporting concrete voids identified in part of Containment Building ring girder 180' Couth section.
(Section II, paragraph 2)
1, The licensec stated that as spoken of in Addendum to Report to DRL

offanuary 19, 1972 a detailed examination will be made of the entire 180 South section when all scaffolding is erected. An engineering

evaluation will be the subject of another Addendum to DRL.

The inspector stated his review of recor'ds for exact location of Cadwell

splices found lacking at the last inspection, indicated those records l
were corrected and now appeared complete.

had been obtained from field records that had not been filed.

The inspector inquired if the licensee was satisfied with an omission of NDT on welded hoop repair replacements for damaged conduit spiral reinforcing.

Observation of Work Performance on First Lif t 30 Segment I, Pour No. 6

The inspector observed concrete testing before placement, aanveyance, delivery, placement, consolidation, finishing and curing of this concrete lift made on 3/23/72. All operations appeared to be in con-formance with job specifications, ACI code 301, FSAR commitment and Repair Procedure CCP-9.

Record Review of North 180 Repair Documentatien (1) The inspector performed an overall review of UE&C's QC records on work completed. The General Files for repairs within each of six concrete placements completed were examined for complete-ness and traceability and, appeared to be without deficiency.
The inspector's comparison of contents of each file de=enstrated consistency and completeness as identified in CO Report No.
50-289/72-03.
(2) The review specifically identified file where previous inspection disclosed some Cadweld splices did not have exact locations. Exam-ination of these records showed that corrections had been made and, this eliminates a deficiency identified at the last inspection.

Seven repert: ccvering thccc items were found without deficiency:

Lack of NDT on Hoop Welds The inspectors examination of records on ring girder North 180

repair, file #D 325 N disclosed that no NDT cnisted on hoop welds.
'
The records were noted "No Radiographs Required". The inspector
'
verified compliance to Weld Procedure No. 22 and referenced code AWS D1.0-69.
Since the latter leaves NDT to job specifications and
'
no NDT was specified on the hoop welds the inspector inquired at Management Interview if licensee accepted the omission.

2.
Inspection of Voids in Ring Girder South 180 l
'
The licensee's Project Manager, Mr. R. Heward, notified Region I on the af ternoon of March 22, 1972, that voids had been identified on
'
Containment Building Ring Girder South 180 section. Inquiry Report No. 50-289/72-01, written by C0 documents this notification and states
that inspect. ion would be made by CO.
The inspector visited the area, where scaffolding recently erected, azimuth 288 clockwise to 243, permitted close inspection on 3/23/72 and observed exposed rebar on 30 Segment III D 101/234 116/249.
Radial anchor
-
tension bars and circumferential bars are completely devoid of concrete on the upper half of ring girder ledge between upper and lower trumplates.
The sides of the upper boxed-out trumplates also exhibit unconsolidated (popcorn) concrete and voids were observed in back of the upper trumplates.
Continuing counterclockwise to Segment II, D 117/133, in about half of this segment the concrete ledge beneath these upper trumplates, the radial anchor bars are exposed. Polaroid photographs taken of the voids in Segment III retained in. Region I files are available upon request.
In telecon with J. Wright on 3/30/72 the inspector was informed that
scaffolding was erected for about 80 on South 180 and no additional voiding was observed. Wright stated that a thorough inspection had not yet been undertaken.
