On September 1,1972 a 400-pound steel plate was accidentally dropped into the Unit No. 2 reactor pressure vessel. The interior of the vessel was supposedly dry, but actually contained 10 inches of '

water.
Damage to the vessel from the falling steel appeared minor

but the investigation is not yet complete.

On September 25, 1972 a bc=b threat was received at the jobsite.
No explosives were located.

The inspector stated that the inspection reporting precedure used by the Directorate of Regulatory Operations had been revised. The
details of the new procedure had been explained to the applicant 's Quality Assurance Manager and would be used starting with this in-G "~
spection.
The applicant acknowledged this information.

The inspector ststed that in this inspection he had attempted to

resolve some of the deficiencies which had been identified in previous inspections but that little progress had been made.
It was observed that no listing of items nor the status of their correction was being maintained at the site and there appeared to be no program to resolve identified deficiencies.
The inspector had spent some time with the applicant 's quality assurance personnel in the compilation of this list and its importance was stressed.
It was explained that the applicant's listing should include all items identified as nonconformances or noncompliances in the Reg-ulatory Operations letters to the. applicant', as well as all deficiencies

reported to the applicant at the management interview which followed each inspection.
Items which had been resolved were specifically

mentioned by the inspector at each management interview and should be deleted from the list.
The applicant stated that a list would be maintained in the future and that added effort would be applied to correct identified items.
