Inspection No. 50-278/81-28 This refers to Mr. M. J. Cooney's letter of November 13, 1981 which transmitted the report of your investigation of the 14 rem exposure to the radiation exposure monitoring badge worn by one of your employees during August 1981.

This incident was investigated by an inspector from the Region I office on October 13-15, 1981.

We have carefully reviewed your investigation report. The review indicates that subsequent to our onsite investigation, your staff learned that the employee's badge had been lost during part of August 1981. The new information is not consistent with that provided to us by the worker during our investigation, in that we were told that his badge had not been lost or misplaced during August 1981.

Our review of the data provided in your report indicates that the maximum possible dose to the badge during the loss period could account for no more than one rem out of the reported 14 rems.

To permit us to complete our assessment of the exposure, please provide us,

l within 30 days of the date of this letter, your determination of the maximum l

exposure the employee could have received during the following periods:

August 15, 1981 2:30 p.m. - 5:00 p.m.

August 17, 1981 4:30 p.m. - 6:00 p.m.

Wherever possible, the determination of this exposure should not rely soley on information provided by the employee.
