Inspection on September 24-25 and 28, 1979 (Combined Report Numbers 50-277/
79-24 and 50-278/79-26)
Areas Inspected:
Routine, unannounced inspection by one regional based inspector of work activities related to concrete expansion bolt anchors in pipe support base plates under Bulletin 79-02.
The inspection involved twenty (20) hours on site by one regional based inspector.
Results: Of the area inspected, no items of noncompliance were found in Unit 2, and one item of apparent noncompliance was identified in Unit 3, as failure to initiate corrective action on an identified nonconformance (Infraction).

2.
Procedure Review The inspector reviewed special procedures developed by the licensee for the implementation of corrective action for IEB:79-02.
The following procedures were reviewed:
(i)
Procedure No. 7902-1, " Procedure for Identification, Inspection, and Testing of Concrete Expansion Bolts for Pipe Supports as Requested by NRC," Revision 1, June 21, 1979; Revision 2, September 10, 1979; Revision 3, September 20, 1979.
(ii)
Procedure No. 7902-2, "For Implementation of NRC Bulletin 79-02, Revision 1, June 21, 1979," issued July 12, 1979.
The inspector did not identify any items of noncompliance in the area reviewe,

3.
Review and Audit of Completed Work The inspector reviewed the records of completed work for anchor boir.1:

0365 33HB-H266 S/S 0369 33HB-H233 S/S 0453 48GB-S14 S/B During the review of the records the inspec;or observed that the base plates for Hanger Nos. 33HB-H267 cnd 33HB-H266 did not conform to the bolt pattern specified for the base plates on drawings 1S0-3369 and 3370, respectively. This nonconformance was identified and reported by QC to PECo Engineering on August 21, 1979, however, no resolution of this nonconformance or any corrective action was initiated.
This is a violation of 10 CFR 50, Appendix B, Criterion XVI, " Corrective Action."
4.
Qualification of Inspection Personnel Engaged in 79-02 Test Program The inspector reviewed the qualification and certification records for QC inspectors engaged in inspection of w]rk under Bulletin 79-02.
Quality Control and inspection is provided by the licensee's subcontractor on site.
The inspector reviewed certification records of 5 QC inspector on a random sample basis.
The inspector observed that out of a sample of five, four inspector's certifications as a Level II inspector in the areas of mech-anical, (Welding and NDE), was not in accordance with the requirements set forth in ANSI N45.2.6-1973.
However, this item was corrected prior to the completion of the inspection.
This nonconformance did not affect any safety-related work.

5.
Reporting and Resolution of Hanger Abnormalities The inspector reviewed the process of supporting and resolution of hanger abnormalities identified during inspections pursuant to IEB 79-02.
This review consisted of a selective examination of documentation, reporting of abnormalities, and the responses provided by the A/E for their resolu-tio.

6.
Witnessing of Test The inspector witnessed torque listing of concrete expansion bolt CEB-0269 on Hanger #333-HB-5140. The test procedure used by the testing crew was as specified in the licensee's Procedure No. 7902-1, Revision 1.
No items of noncompliance were identified in this area.
7.
Exit Interview The inspector met with the licensee representatives (denoted in paragraph 1)
at the conclusion of the inspection on September 28, 1979.
The inspector sunmarized the purpose, scope and findings of the inspection.
