NRC INSPECTION ~ REPORT NOS. 50-275/93-16 AND 50-323/93-16 This refers to the routine inspection conducted by M. Miller and F. Gee during the period from May 25 through July 6,1993. This inspection examined your activities as authorized by NRC License Nos. DPR-80 and DPR-82. At the conclusion of the inspection, the inspectors discussed their findings with members of the PG&E staff.
Areas examined during this inspection are described in the enclosed inspection report. Within these areas, the inspection consisted of selective examinations of procedures and representative records, interviews with personnel, and observations by the inspectors.
Based on the results of this inspection, it appears that certain areas of your
,
activities were not conducted in full compliance with NRC requirements, as set forth in the enclosed Notice of Violation (Notice). These violations involved (1) two instances of inadvertent dilution of boron in the reactor coolant i
system, resulting from failure to follow or provide adequate procedures; and (2) failure to perform timely reviews of action requests to determine whether a quality evaluation was requirea. The boron dilution events represented a failure of plant processes to properly control reactivity due to poor procedure quality and adherence. We are also concerned regarding the length of time that passed after the initial event before plant management initiated corrective action.
