on January 26, 1989-, at the Diablo Canyon'ite.. Mr. Faulkenberry opened the meeting by introducing the topics for discussion:

the numerous recent incidents of operations errors and valve misalignments,'he configuration management and,system engineering programs, and the licensee's pursuit of the industry's rising standard of excellence.

With regard to'numerous recent incidents of operations errors and valve misalignment, Mr. Faulkenberry noted that while in most cases the resultant events were not of major significance,'he overall number and frequency of such errors ha's caused this to develop into an item of principal concern.

Some of these problems are described in the two most

'ecent resident inspection reports, issued November 23 and December 20, l988, and a draft inspection report covering the period through January 21, 1989, 50-275/88-32 and 50-323/88-30.

Mr Faulkenberry acknowledged the licensee's response to the November 23 inspection report, submitted January 6,

1989, regarding instilling. proper instincts in'personnel,

'and asked Mr. Townsend what, if any,'mprovements had been seen.

Mr. Townsend stated that at this time it was too ea> ly to see progress.

However, he noted that they were actively pursuing the actions described in their letter to achieve improvement in personnel performance through (1)

"unrelenting" communication of management goals and expectations, and (2)

personal accountability.

Mr. Faulkenberry stated that Region V will continue to monitor PGSE's activities in this area and would expect to see progress, i.e.,

a significant reduction in personnel errors, within the next few months.

Following some discussion of the action items in the January 6 letter, Mr. Faulkenberry initiated discussion of the licensee's progress with respect to the configuration management and System Engineering programs.

Mr. 'Faulkenberry noted that he had reviewed the licensee's policy statement on configuration management and found that while it provided a good foundation, it lacked specificity.

Additionally, outside of the response to the Maintenance Team inspection report (October 5, 1988),

he noted that the Region had received very little specific information as to the content and actual expected end products

of the Configuration Management program.

Also, Region V has not received commitments as to when certain end products such as individual system design documents wi 11 be completed.

Mr. Tresler noted that the program had been discussed in a December 9, 1988, meeting in Walnut Creek; however, he stated that he would make arrangements to fully brief Region V senior management, on all aspects of the program, within the next three to four weeks.

With respect to the system engineering program, Nr. Faulkenberry noted that PGSE's progress in developing a fully functional Systems Engineering program, staffed with fully'rained and fully qualified systems engineers who are actively involved in all activities related to their systems, appears to be slow.

He also expressed concern that a

recently issued System Engineering Program procedure (AP A-350)

identified a number of responsibilities without establishing clear support functions.

(As an example, although the system engineer is required to review operating procedures, they are not in the review chain for these procedures).

In addition, the procedure did not provide details of the on.-the-job training requirements.

Mr. Tresler stated that while on-the-job training would be she core of the system specific training, a supplemental training program was under development and scheduled to start in February which would cover topics such as design basis concerns, safety evaluations; and surveillance testing.

Nr. Faulkenberry stated that Region V will continue to evaluate progress in this area.

Nr. Faulkenberry then discussed the licensee's pursuit of the industry's rising standard of excellence and encouraged the licensee to apply its resources to be an industry leader.

Nr. Townsend indicated that PGLE continues to pursue leadership in many industry issues.

In addition, he invited Region V management to spend more time at Diablo Canyon.

Mr. Faulkenberry noted that the plant material conditions appear to have degraded since his last visit.

He noted that he had observed accumulations of oil on pump skids and around the diesel generators and tool bags and accumulations of miscellaneous materials in various equipment rooms with no evidence of ongoing work.

He added that the cond>tions noted were below par for other reactor sites in the Region.
