SUBJECT:
PALISADES INSPECTION REPORT 50-255/98010(DRP) AND NOTICE OF VIOLATION
Dear Mr. Palmisano:
On July 1,1998, the NRC completed an inspection conducted from May 7 through July 1,1998,
. at your Palisades Nuclear Generating Plant. The areas examined during the inspection are identified in the enclosed report. The inspectors noted improved performance, as compared to past refueling outages, pertaining to the implementation of your switching and tagging order program. Operator performance was also good, including good compliance with the shutdown operations plan during the outage. During the inspection period, a misalignment of a single control rod occurred during low power physics testing. While the rod misalignment exceeded procedural requirements, the consequences of the rod misalignment were minor. Nonetheless, the incident was significant because operators are required to perform self-checking during the conduct of a reactivity manipulation. A secondary issue with the event was the recurrence of a problem with the Primary Rod Position Indication System (PIP).' The problem with the PIP delayed the receipt of the four inch and eight inch deviation alarms.
Based on the results of this inspection, the NRC determined that a violation of NRC requirements occurred regarding inadequate equipment control. The violation is of concern because it
involved proper plant configuration not being maintained while filling the safety injection tank bottles, and the establishment of a flow path allowing approximately 1200 gallons of water to be pumped from the refueling cavity to the containment sump with the shutdown cooling system in operation.

This violation is cited in the enclosed Notice of Violation (Notice) and the circumstances surrounding the violation is described in detail in the enclosed re sort. You are required to respond to this letter and should follow the instructions specified in the enclosed Notice when preparing your response.= The NRC will use your response, in part, to determine whether further
. enforcement action is necessary to ensure compliance with regulatory requirements.
