Elrnd, Section Chief Division of Reactor Projects SUMMARY Scope: This special report concerns the Enforcement Conference held with licensee representatives in the NRC's Region II office on January 31,.1986 to discuss the repetitive failure of the stop check valves in the steam supply system to the auxiliary feedwater (AFW) pump turbines. The details surrounding these failures were addressed in Inspection Report 50-250,251/86-0 Results: Two Unresolved Items were determined to be violations; AFW Limiting Condition for Operation exceeded due to inadequate management contrni (250,251/

86-11-02) and inadequate documentation to justify continued operation with 1 missing valve parts in the AFW system (250,251/86-11-03) (paragraph 3). f l

Enforcement Conference An Enforcement Conference was held on January 31, 1986, in the Region II office with the persons identified in paragraph 1. The topic of discussion concerned the repetitive failure of the stop check valves in the steam supply system to the AFW pump turbines. The details surrounding these failures were discussed in Inspection Report 50-250,251/86-0 During the discussion, the licensee addressed the management and technical issues related to the AFW valve failures and deficiencies identified during the NRC inspection conducted from January 6-10, 1986. The licensee discussed the proximate and root causes of the inspection findings and the corrective actions being taken to resolve the deficiencies and prevent their recurrenc The licensee's statement that they believed the valve failures did not constitute a loss of safety function is being evaluated by the NR . Resolution of Previous Unresolved Items Unresolved Items (UNRs) are matters about which more information is required to determine whether they are acceptable or may involve violations or deviation Four UNRs were identified in paragraph 5 of Inspection Report

2 50-250,251/86-0 These issues were discussed with the licensee during the Enforcement Conference on January 31, 1986, and are resolved as follows:

(CLOSED) UNR 250,251/86-02-01 - Resolve NRC Concerns in the Use of Weaker Material for AFW Valve Repair. The licensee stated at the Enforcement Conference that although the new valve discs used in the repair of the AFW valves were of lower tensile strength than the original discs, they still met the design requirements for this applicatio (CLOSED) UNR 250,251/86-02-02 - Resolve NRC Concerns Over Licensee Actions to Obtain a More Permanent Solution for AFW Valve Problem The licensee's long range plans for a permanent solution to the AFW valve problems will be tracked as Inspector Followup Item 250, 251/86-11-0 (CLOSED) UNR 250,251/86-02-03 - Resolve NRC Concerns Over no Actions Being Taken When Damage to Three AFW Valves was Reported to Responsible Personnel. On January 2,1986, radiographers identified three AFW steam supply stop check valves as having missing or misaligned disc assemblies / guide stud This finding was verbally relayed to the licensee's engineering staff and should have resulted in a declaration of valve and AFW system inoperability as required by Non-Conformance Report (NCR) 86-00 A breakdown of management controls in the review of NCRs and poor judgement on the part of the engineering staff, however, allowed Units 3 and 4 to continue operating (for almost five days and four days, respectively) beyond the time permitted by the Technical Specification Limiting Condition for Operatio The UNR has been resolved as Violation 250,251/86-11-02, ' Exceedi ng Limiting Condition for Operation Due to Inadequate Management Controls in Properly Determining Operability of the AFW Syste (CLOSED) UNR 250,251/86-02-04 - Resolve NRC Concerns Over the Inadequate Search for the AFW Valve Missing Parts. All 12 AFW stop check valves were disassembled and repaired under NCR 85-206 (and related Plant Work Orders) during November and December 1985. The NCR directed that all valves in the flow path from the stop check valves to the AFW turbines be radiographed in order to locate three missing disc guide studs and a missing thrust washer. The missing parts were never found and both units were returned to service.
