Millstone Nuclear Power Station, Unit ? Response to I&E Inspection No. 50-245/85-28 Pursuant to the provisions of Section 2.201 of the NRC's Rules of Practice, Part 2, Title 10, Code of Federal Regulations, this report is subnitted in reply to Feference (1).

Reference (1) infonned the Northeast Nuclear Energy Conpany (NNECo)

of a Severity Iavel IV violatio Violation As a result of the inspection conducted on tbvenber 5-8, 1985, and in accordance with the NRC Enforcenent Policy (10CFR2, Appendix C),

the following violation was identified:

Technical Sepcification 6.11 requires that procedures for radiation protection be established, implenented and adhered t Procedure SHP 4912, Revision 6, Section 4 requires, in part, that workers be responsible for "... reading, under-standing, initiating, and following RWP instructions".

. -2-a Contrary to the above, on October 27, 1985, two maintenance workers entered a radiological area in violation of the requiranents listed on the controlling RWP #3344. Specifi-cally, the workers performed work in the reactor cavity without wearing respiratory protection equipnent as required by the RWP. One individual of the working party consequently sustained a limited intake of radioactive materia h is is a Severity Level IV violation (Supplement IV).

Response A memo has been issued to all Health Ihysics personnel which provides formal guidance on making field changes to the radiological controls specified on radiation work permits. All changes to the radiological controls listed on a radiation work permit will require the approval of the Assistant Radiation Protection Supervisor /
