This refers to the special safety inspection performed by an NRC Augmented Inspection Team (AIT) comprised of personnel from NRC Region III and the Office of Nuclear Reactor Regulation headed by Mr. J. H. Neisler of this office, on J~nuary 17 through January 20, 199 The purpose of this inspection ~as to review the circumstances surrounding the Unit 2 reactor trip and loss of off-site power on January 16, 1990, at Dresden Station Unit 2 authorized by NRC Operating License No. DPR-1 This also refers to the discussion of our findings'
with Mr. N. J. Kalivianakis and other members of your staff at the conclusion of the special inspectio The enclosed copy of our AIT report idtntifies areas examined during the inspection~ Within the~e ireas, the inspection consisted of a selective examination of equipment, procedures.and representative records, observations
. and interviews with personne During th~ inspectio~, the AIT noted that certain balance of plant (BOP)
equipment has not received adequate preventive maintenanc The 213 shutdown cooling water discharge valve had riot been included in the preventive maintenance program at.the time of its failure and preventive mainte~ance had not been performed on the 2D Condensate/Booster.Pump motor when baseline predictive data indicated that the motor condition was inferior to the other condensate/booster pump motors. While you have undertaken some predictive and preventive maintenance initiatives for BOP equipment over the past* several years, these failures give rise to general concerns about the adequacy of your programs for BOP equipment that can affect or challenge safety-related system We request that you be prepared to discuss tha status and scope of your BOP preventive maintenance program at the next monthly management meetin We understand that you have not completed the investigations to determine the cause of the fire in the 20. Condensate/Booster Pump motor and the failure of
