b Inspection Summary Inspection on May 22, 1979 (Report No. 50 10/79-11: 50-237/79-14_1 50-249/79-12)
Areas Inspected: Routina, unannounced inspection of the response of the licensee's organization during a scheduled drill involving simulated contaminated injaries.
The drill was conducted by the licensee's Emergency Medical Contractor.
The irspection involved 16 inspector-hours on site by 4 NRC inspectors.
Results: For the areas inspected, no items of noncompliance or deviations were identified.

Drill The inspectors observed the response by the licensee's organization and selected offsite s2pport agencies during a scheduled drill conducted by the licensee's emergency medical contracter, Radiation Management Corporation, on May 22, 1979.
The drill included a training exercise for hospital and site personnel.
Site personnel and the ambulance crew responded to the simulated accident and the hospital personnel were guided through medical treatment and decontamination procedures.
Radiation Management Corporation personnel prepared the victim and supervised the hospital segment of the drill.

Action at the Scene of the Accident
.
The drill consisted of two plant employees being injured and contaminated when " c transfer hose transferring miscellaneous Waste Evaporator Bottoms ruptured.
The hose struck one operator on the forehead and knocked him unconscious; both he and his wound became contaminated. Operator #2 slipped and

fell, rushing to aid Operator #1, cutting and contaminating his arm.
Operator l12 gave emergency first aid to Operator #1 until help arrived.
The initial emergency respcse came from a
two Rad Chem technicians with a first aid kit and a survey meter.
The inspectors and the drill observers noted that it took about 8 to 10 minutes for the rescuers to move the victim out of the relatively high radiation area of 4000 cr/hr @
contact er.d 150 mr/hr @ 5 feet.
The observers also noted that it took approximately the same amount of time to cover the unconscious victim with a blanket.
The Coal City ambulance arrived in approximately 15 minutes.
Communications, which were all done by radio, seemed to go very well in spite of the loud turbine noise in the area.
A Rad Chem technician accompanied the injured personnel to the hospital.

Control Room Actions The inspectors not. that control room personnel verified the injuries and the contamination of the victims, asked if they could be moved tnd then notified the shift fore =an.
Rad Chem was notified that first aid and contamination controls were needed at the accident site.
Communications were noted to be very good between the control rcou and the rescue team.
Control room personnel called the Coal City Rescue Squad (ambulance), the hospital emergency room and gave the ER personnel injury and contamination status.
They also alerted the gate house that the ambulance was coming and notified the physician of the injuries and contamination.
DOE, Chicago, was notified of the drill.
c.
Pospital Act ions Hospital ER was notified of the two injured / contaminated persons.
Hospital security was called and they began to set up the room.
The hospital Nuclear Medicine staff provided additional survey capability.
Emergency Room personnel dressed ort two nurses and Dr. Tarizo.
The inspectors noted that the ER was notif ed of the estimated time of arrival of the embulance by the plant by phone and from the ambulance b, radio.

the night shift were held over to participate in this drill as a training session. All previous drills have involved only day shift personnel. With the exception of a few minor errors in the removal of simulated contaminated clothing, the ER personnel performed well.
A critique was held with hospital and plant personnel as

noted in Paragraph I following the drill.
The video tapes were reviewed and comments of NRC, RMC, and the participants in the drill were discussed at length.
Significant items were noted by the medical consultant for future implementation.
