Licensee Action on Previous Inspection Findings (Closed) Unresolved Item (050-010/77-12): Conflict between Section VIII and Section XIV of the W. A. Randolph, Inc.'s (WARI) QA Manual. The inspector reviewed the QA Manual and established that the manual has been revised to provide clarification.

PT indications on the primary steam drum safety valve nozzles. The inspector reviewed reports and determined that the indications were removed and no indications were found during subsequent liquid penetrant examinations.

Review of ISI Records The inspector reviewed six ultrasonic examination, two liquid penetrant examination and one visual examination reports. The

reports reviewed were piping welds in the recirculation system,

primary feedwater system, primary steam system, steam generator

,

supports, reactor pressure vessel studs and reactor pressure vessel to flange welds. The inspector determined that:

(1) the reports were reviewed by the licensee and the authorized inspec-tor, (2) the applicable procedural requirements were met and (3)

no defects were identified. No items of noncompliance or devi-ations were identified.

Review of Concrete Placement Records i

The inspector reviewed concrete placement records for the north half of the HPCI building base mat.

The records reviewed included

QA checklist, batch tickets, final inspection checklist for rein-forcing steel and seven day cylinder test reports. The inspector determined that:

(1) the records met the applicable procedures and specifiertions requirements and (2) checklists were properly signed-off and reviewed. No items of noncompliance or deviations were identified.

Exit Inte rview The inspector met with the licensee representatives at the conclusion of the inspection. The inspector summarized the scope and findings of

the inspection. The licensee acknowledged the information.
