MEMORANDUM FOR: E. L. Jordan, Director, Division of Emergency Preparedness

& Engineering Response, Office of Inspection and Enforcement FROM: C. E. Norelius, Director, Division of Reactor Projects, RIII

SUBJECT:

PROPOSED IhTORMATION NOTICE: VARIOUS DISCRIPANCIES BE*NEEN AS-BUILT CONST(UCTION DRAVINGS AND AS-BUILT INSTALLATIONS IN THE ELECTRICAL AND INSTRUMENTATION AND CONTROL AREAS l l

Pre-licensing as-built inspections at Fermi Unit 2 by RIII resulted in the identification of numerous discrepancies between .2s-built documents and the <

as-built configurations in the electrical and I&C areas. These findings delayed .i licensing of the plant and will have an ongoing impact on operation and mainte- j nance activities until the first refueling outage. We believe this issue merits j dissemination to other CP and OL holders. Enclosed is a proposed Information  :

Notice for your review and consideration.

If you have any questions concerning this matter, please contact Cordell Williams N (ITS 388-5598) of the Division of Reactor Safety, RIII. f i

.', , f3 DI DRAFT IE Information Notice No.  : Various discrepancies between as-built construction drawings and as-built installations in the electrical and instrumentation and control areas n.

Addresses:

All licensees of nuclear power reactor facilities holding an Operating License (OL) or Ccnstruction Permit (CP).

Purpos e :

This information notice is provided to alert licensees and applicants to defi-ciencies identified by the NRC during the construction of Fermi Nuclear Power Plant, Unit 2, regarding as-built construction drawings not correctly reflect-ing as-built electrical and Instrumentation and Control (I&C) installations.

These deficiencies included discrepancies between different construction drawings for the same installation or activity. Some of these findings were found to directly affect plant operations and maintenance, and contributed to delays in -

the licensing of the plant. Recipients are expected to review the information j for applicability to their facilities and consider actions, if appropriate, to ,

preclude similar problems from occurring at their facilities. Holders of OLs '

should review these findings in the context of additions or modifications to i existing plant installations. However, suggestions contained in this information  !

notice do not constitute NRC requirements; therefore, no specific action or written I response is required. i Description of Circumstances '

During routine NRC pre-licensing inspections of as-built design and construction drawings / specifications to determine if they correctly reflected the as-built condition of the plant, discrepancies were identified in the electrical and I&C areas. The initial indication of what was later determined to be a rather large problem was the NRC determination that installed fuses and thermal overloads did not match installation drawings. Later, the magnitude of the problem was revealed by numerous instances where either as-built drawings castradicted each other (i.e., disagreement between single line, schematic and wiring diagrams) er did i not reflect as-built conditions. Construction and preoperational testing did not identify any of the discrepancies.

In response to the NRC findings, the applicant completed an approximately 100% ,

reinspection of electrical and I&C installations. The applicant's inspection .;

effort resulted in the identification of over 7300 discrepancies and errors i between as-built field configurations and associated design and construction '

drawings and specifications. The nature of these discrepancies are summarized f below: i i

I l-t l

l . 1

',- DRAFT Discrepancies which, if left uncorrected, could result in the less or incorrect function of a safety-related component or system. (154 deficiencies)

Examples:

. Tubing connections wrong Discrepancy in internal wiring that may jeopardize circuit integrity

. Unidentified jumpers installed

. Wrong component installed Indicating lights wired incorrectly Discrepancies which, if left uncorrected, could result in the impaired performance of operations and maintenance personnel.

. Correct design documents, but deficient hardware.

(300 discrepancies) /

Examples:

. Missing shield to ground connection

. Jumpers across overload heaters Spare wires not disconnected to active component and not shown on drawing Hardware different than shown on drawing

. Wrong relay nameplate

. Missing hardware i

j Correct as-built hardware, but deficient drawings.

(1900 discrepancies) h i

Examples:

Additional wires exist but are not shown on wiring

! diagram Physically there are two wires on active device but

  • i wiring drawing shows three wires Drawing shows jumpers bat jumpers are not installed Incorrect cable number on drawing

. Other minor hardware or drawing discrepancies. (5000 discrepancies)

Examples:

Devices not located exactly per the drawing Missing and/or incorrect Brady tags on switchgear, I&C cabinets, etc.

Wiring on correct terminal block; however, left hand side i

or right hand side termination does not agree with print.

Relay sockets do not look like layout drawings.

2 '

.s

. DRAFT Wiring drawings show letters (B, W, R, G, etc.) for wires whereas physically the wires are marked with numbers (1, 2, 3, 4, etc.) t Permanent tags not on power supply feeder positions.

Entensive actions by the applicant were necessary to correct the most significant discrepancies prior to issuance of an operating license by the NRC.

Discussion i

For the safe operation and maintenance of a nuclear power plant it is necessary that all as-built design and construction drawings be completed and verified to match appropriate installations, prior to issuance of an operating license.

10 CTR 50 Appendix B, NUREG-0800, Rev. 2, Standard Review Plan Section 17.1, ANSI N45.2-1977 Section 7, ANSI N18.7-1976 Section 5.2.15, as applicable, ,

j identify requirements and measures to be taken by licensees to control docu-j ments for the safe operation and maintenance of a plant. '

Because construction and design drawing deficiencies, if uncorrected, could adversely affect operational safety, recipients of this notice should review the information presented to avoid similar deficiencies at their facilities. I No specific action or written response is required by this information notice.  !

If you have any questions about this matter, please contact the Regional Administrator of the appropriate NRC Regional Office or this office. '
