Title | Failure to Resolve Degraded Conditions in a Timely Manner |
Description | The inspectors identified a noncited violation of 10 CFR Part 50, Appendix B, Criterion XVI, Corrective Action, for the licensees failure to promptly correct degraded or nonconforming conditions in that the conditions were not corrected at the first available opportunity or appropriately justify a longer completion schedule. Some examples of affected degraded or nonconforming conditions included degraded atmospheric relief valve discharge line silencer, essential service water system water hammer events and internal corrosion, and 23 items on the Operability Evaluation Database that had not been corrected prior to the start of the last refuel outage. As corrective actions for this issue, the licensee implemented interim procedural guidance and initiated Condition Report 27071 to evaluate the adequacy of tracking methods used for degraded, nonconforming, or unanalyzed conditions. In addition, the licensee initiated a review of work requests, condition reports, and other items for degraded, nonconforming, or unanalyzed conditions and is assessing the justification for delayed implementation of these corrective actions.
This issue was more than minor because it affected the equipment performance attribute of the Mitigating Systems Cornerstone objective to ensure the availability, reliability, and capability of systems that respond to initiating events to prevent undesirable consequences. Using Inspection Manual Chapter 0609, the issue is determined to have very low safety significance because the finding is not a design or qualification issue confirmed not to result in a loss of operability or functionality; did not represent an actual loss of safety function of the system or train; did not result in the loss of one or more trains of nontechnical specification equipment; and did not screen as potentially risk significant due to a seismic, flooding, or severe weather initiating event. The inspectors determined that the finding has a crosscutting aspect in the area of human performance associated with the component of resources because the licensee failed to provide adequate procedures to assure timely resolution of degraded or nonconforming conditions H.2(c). |
