| Title | Failure to Identify Sources of Boron Leakage |
| Description | The inspectors identified a noncited violation of 10 CFR Part 50, Appendix B, Criterion V, Instructions, Procedures, and Drawings, involving the licensees failure to identify sources of boron leakage and document them in a corrective action document. Specifically, prior to October 23, 2009, the licensee failed to accomplish the requirements of Procedure AP 16F-001, Boric Acid Corrosion Control Program, Revision 5, step 6.4.1, which states, in part, Sources of boron seepage/leakage shall be identified/verified and documented in the applicable corrective action document. During a boric acid walkdown, the inspectors identified 11 sources of boron leakage which had not been previously identified and documented by the licensee. The licensee entered this finding into their corrective action system as Condition Report 00021274. The finding was determined to be more than minor because it was associated with the Initiating Events Cornerstone attribute of human performance and affected the cornerstone objective to limit the likelihood of those events that upset plant stability and challenge critical safety functions during shutdown as well as power operations. The inspectors used Inspection Manual Chapter 0609, Significance Determination Process, Attachment 4, Phase 1 Initial Screening and Characterization of Findings, and determined the finding was of very low safety significance (Green) because the issue would not result in exceeding the technical specification limit for identified reactor coolant system leakage or affect other mitigating systems resulting in a total loss of their safety function. The inspectors also determined that the finding had a crosscutting aspect in the area of problem identification and resolution, operating experience, where the licensee did not institutionalize operating experience through changes to station processes, procedures, equipment, and training programs [P.2.(b) |
