Plant Systems - Control Room Emergency Air Conditioning System -
This report was initiated by Incident Report 83-061.
CONDITIONS' PRIOR TO OCCURRENCE:
Mode 6 - RX Power 0 % - Unit Load 0 MWe, DESCRIPTION OF OCCURRENCEt At 1520 hours0.0176 days <br />0.422 hours <br />0.00251 weeks <br />5.7836e-4 months <br />, April 1, 1983, the Operations Surveillance Coordinator discovered that testing of the Control Room Emergency Air Conditioning System had not been performed as required by Technical Specification Surveillance Requirement 4.7.6.1a. Surveillance of the system is required monthly, and had been overlooked since shutdown of the plant for refueling in January, 1983. The Shift Supervisor was immediately notified and the required testing was immediately performed. Results of the testing were satisfactory, demonstrating the system was operable throughout the period involved.
APPARENT CAUSE OF OCCURRENCE:
The oversight was discovered by the coordinator when he noted that the Salem Unit 2 Technical Specifications required operability of the system in all modes (vs. the Unit 1 Specifications which only require operability in Modes 1-4). The Inspection Order (I/0) System card requiring performance of the surveillance for Unit 2 was written at the same time as the one for Unit 1 (January 1981), and the Unit 1 applicability requirements were erroneously included. The related surveillance procedure reflected the I/O card requirements.
ANALYSIS OF OCCURRENCE:
The operability of the Control Room Emergency Air Conditioning System ensures that the ambient air temperature does not exceed the rating of the equipment and instrumentation cooled by the system and that the room will remain habitable during and following all credible accident conditions. The operability requirements are consistent with the design criteria of 10 CFR Part 50.
As noted, subsequent surveillance demonstrated that the system was in fact operable. The occurrence therefore involved no risk to the health or safety of the public. The event illustrated inadequacies in

- ANALYSIS OF OCCURRENCE: (cont'd) the implementation of procedural controls which threatened to cause a loss of redundancy provided in an engineered safety feature system.
As such, it is reportable in accordance with Technical Specification 6.9.1.9c.
CORRECTIVE ACTION:
As mentioned, the surveillance was immediately completed, and results were satisfactory. The I/O card and surveillance procedure were revised to reflect Technical Specification requirements. A review of Units 1 and 2 Technical Specifications was performed to identify any other similar problems, with none found. Further study of the specifications will be completed to determine if the respective applicability requirements are satisfactory. A License Change Request will be initiated as necessary, based on the final results of the review.
