Missed Surveillance CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 Reactor Power 100%

DESCRIPTION OF OCCURRENCE:

When reviewing surveillance records, it was determined the Surveillance Requirement 4.3.3.6.2 was not completed within the required interval.

Surveillance 4.3.3.6.2 requires the circuits between required detector instruments and the Control Room shall be operable at least once per 92 days per approved procedure.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

Personnel error.

A timely review of items identified as outstanding was not completed.

ANALYSIS OF OCCURRENCE:

As a result of the corrective action taken because of license event report 80-21/03L SRO review of the outstanding inspection orders, this item was identified.

CORRECTIVE ACTION

When identified, the surveillance was completed and documented.

A copy of the outstanding inspection orders list will be sent to the Senior Reactor Operators on shift for their review.
