o CAUSE CESCRIPT1oN ANo CORRECTIVE ACTIONS 27 I

o I Leakare path is postulated to have been through the tank relief valve.

gi During filling of the tank, it was pressurized to approx 119 psis, due mI to faulty pressure instru=entation.

It is believed the elevated pressure,

S yI caused leakage through the relief valve which no:: ally lifts at 130 psig.,

The instru=entation has been recalibrated.
