| Title | Failure to Verify the Adequacy of Credited High Energy Line Break Barriers |
| Description | A finding of very low safety significance and an associated NCV of 10 CFR 50, Appendix B, Criterion III, Design Control, was identified by the inspectors when the licensee credited fire doors for High Energy Line Break (HELB) protection without a supporting test or evaluation. Specifically, Procedure 4.02 credited fire doors with protection of safety-related equipment against a HELB when the primary HELB barrier was disabled without a test or evaluation to demonstrate the doors could withstand the HELB environment. This issue was entered into the licensees CAP as CRPLP201500371, NRC Concerns with Calculation EAPSACCWHELB0217, dated January 22, 2015. The performance deficiency was determined to be more than minor because it was associated with the Design Control attribute of the Mitigating Systems cornerstone and adversely affected the cornerstone objective of ensuring the availability, reliability, and capability of systems that respond to initiating events to prevent undesirable consequences. Specifically, the licensee did not have an evaluation to demonstrate that barriers relied upon to protect mitigating systems from a HELB initiating event could perform the credited protection function. The inspectors answered No to the questions in Exhibit 2.A, Mitigating Systems Screening Questions, and as a result determined the issue was of very low safety significance. This finding was not associated with a cross-cutting aspect since the calculation in question was created in 2003 and therefore did not represent current performance. |
