[Z] Temporary Job Site Inspection 31995 Northwestern Highway, Farmington Hills, MI PROGRAM SCOPE This was a limited-scope, reciprocity inspection of a service provider licensed by the State of California. The licensee provided preventative maintenance, installation, and decommissioning services of various gamma stereotactic radiosurge1y (GSR) devices. Licensed activities performed at this particular job site included the initial source loading and quality assurance checks of an Elekta Model 43685 Leksell Gamma Knife Icon' device. The licensee filed its reciprocity request with the NRC for this service on April 5, 2019. All activities were performed by two authorized source handlers employed by the licensee. On April 16, the licensee off loaded the cask from the transport vehicle integrated it into the source loading machine. On April 17, the licensee performed the source loading of the GSR devic This inspection consisted of interviews with selected licensee personnel, a review of select records, and observations of licensee staff performing the service activities described above at the customer's facility. The inspector observed licensee staff perform leak tests and surveys of the cask prior to both moving it from the vehicle and opening it for placement into the source loading machine. The inspector verified that the licensee's survey meters and portable wipe counters were calibrated and operable. Each authorized individual wore whole body and ring dosimeters, in addition to a calibrated alarming ratemeter/direct reading dosimeter devic The inspector observed the security of byproduct material, personnel exposure controls, postings, source leak tests, and periodic wipe tests and surveys throughout the source loading. The authorized individuals demonstrated their knowledge of the licensee's operating and emergency procedures, site specific tasks, stopping points, and radiation safety principles while performing the source installation and through interviews with the inspecto The inspector reviewed pertinent documents including leak test records, dosimetry, bill of lading, and the State of California License authorizing the specific individual to perform these services. Independent and confirmatory surveys performed within the GSR treatment room indicated readings below regulatory limit No violations ofNRC requirements were identified during this inspection.
