Background This radiographic operation was being conducted on a jet engine located in the Hawaiian Airlines maintenance shop of the Henolulu International Airport.

The work had been authorized on a Form NRC-241, dated May 1, 1989, in accordance with the general license provision of 10 CFR 150.20.

The Form NRC-241 was acknowledged on May 10, 1989 Ly R. D. Thomas of.the Region V office.

Transportation Requirements

The Inspector examined the shipping papers and the source overpack in accordance with DOT /NRC requirements.

The shipping papers were complete i

and acceptable. _The source overpack was properly identified and was labeled with a yellow two D0T label.

No violations were identified.,

4.

Posting of Area

,

At the time of the inspection, the source was not exposed; therefore, signs and barrier ~were not in place.

A discussion with the radiographer indicated that the required signs and~ barriers were present, and had been used during the earlier exposure -of the source.

The utilization log

5.

Personnel Dosimetry The Inspector observed that the radiographer and the helper had been assigned personnel dosimetry.

Both individuals were wearing a film badge or TLD and pocket dosimeters.

6.

Survey Instrument The Inspector observed that tbe radiographer had a radiation survey for use.

The instrument was in current calibration and operable.

No violations were identified.

Surveys An examination of the utilization log indicated that the proper surveys had been conducted while the source was exposed.

The area was being

Radiographic Exposure Device

The exposure device in use was a Gulf Nuclear Model 20V.

The device was properly identified and labeled to indicate the presence of an iridium-192 source of approximately 60 curies. The device appeared to be secured since the locking mechanism was engaged and the key had been removed from the lock box.

The guide tube was in place and positioned in the jet engine housing.

The drive cable was attached and positioned on the ground with approximately 25 to 30 feet between the exposure device and the cranking handle.

No violations were identified.

11.

Conclusion The Inspector reviewcd the findings with the radiographer.

He was told that no violations were identified and the operation was quite acceptable.
