I~~gog Stone.&Webster'.sFieldQualityControlgroupinaccordancewiththeirQualityVerificationofGraver'sQualityProgramissuedsevenunsatis-factoryInspectionReportsandtwoNonconformanceandDispositionReportsduringafiveweekperiodendingDecember15,1977.Areviewofthesereportsrevealedthefollowingdeficiencies:A..Contraryto10CFR50AppendixBCriterionVI(DOCUMENTCONTROL)work,wasbeingperformedutilizingaprocedure,fortemporaryattachmentremovalthat,hadnotbeenapprovedbyStoneandWebsterEngineering.B.Contraryto10CFR50AppendixB.CriterionIX-(CONTROLOFSPECIALPROCESSES)aweldingprocedurewasbeingusedthatwasnotqualified.fortheapplication(i.e.,weldingwasbeingperformedoncladdingthinnerthanspecifiedintheprocedure).Inaddition,required,nondestructivetestingwasnotbeingperformedtoverifydefectremovalpriortowelding.C.Contraryto10CFR50AppendixBCriterionX(INSPECTION),Graver'sQualityControlgroupwasnotdocumentingtheirinspection(reviewandacceptance)ofrepairweldsperformedasaresultofremovaloftemporaryattachments.D.Contraryto10CFR50AppendixB.CriterionXVI(CORRECTIVEACTION),measureswerenotinitiatedtocorrectconditionsidentifiedbyStoneandWebsterFieldQualityControlaboveasbeingadversetoquality.After.theyhadbeeninstructedtodosobyStoneand.WebsterEngineering,Graverdid.notdocumenttheirinspectionofrepairwelds,usetheprocedureidentifiedbyStoneandWebsterasapplicableforclad'lessthan1/16"thick,andtheydidnotfollowtheirprocedurefortemporaryattachmentremovalasapproved.II.SAFETYANALYSISTheprimarycontainmentcladmaterialanditsfunctionaredescribedinSection5.0ofthePreliminarySafetyAnalysisReport.Thecladformsanintegral,partofthebasematerialoftheprimarycontainmentliner.Thecladsurfaceisdesignedto.providecorrosionprotectionofthebasematerialandpreclude.contamination.fromthesuppressionpoolwater.Thecladdingisnotrequiredfor.thedesignstrengthorleakintegrityoftheprimarycontainmentliner.Theweldrepaircavitiesdidnotexposethecarbonsteelbasemetal.Therefore,this,doesnotresultinasafetyhazardtoNineMilePointUnit2.Theweldrepairsdonotjeopardizethecorrosionresistantperformanceofthecladortheprimarycontainmentlinerdesignrequirements.

E~f III.CORRECTIVEACTIONThefollowingactionshavebeentakentocorrectthedeficienciesidentifiedinSectionI:A.AprocedurefortemporaryattachmentremovalhasbeenreviewedandapprovedasofNovember22,1977.B.Stone.andWebsterhasreviewedtheapplicabilityofweldingprocedures.forcladsurfaceweldrepairs.TheappropriateweldingprocedureinquestionwasfoundacceptableasitwasinaccordancewiththeASMEcoderequiredbypurchasespecification.Also,cladsurfaceweldrepairswereallliquidpenetrantinspectedandaccepted.C.Theprocedurefortemporaryattachmentremovalprovidesfortheinitiationofatemporarywelddatasheetto,documenttheweldprocedureusedandQualityAssurancesign-off.D.OnJanuary4,.1978,ameetingwasheldatStoneandWebster'CherryHillOperationsCenterwithGraverManagement.todiscussthese'eficiencies.Asaresultofthismeeting,thefollowingactionshaveorwillbetaken:1.GraverhaspreparedasupplementtotheNineMileQualityAssuranceManualwhich,hasbeenreviewedandaccepted.Itisanticipatedthatthissupple-mentwillbeissuedshortly.Thissupplementestablishesasystemtoassurethatconditionsadversetoqualitysuchasaudit,findings,deficiencies,deviationsfromprocedures,failures,malfunctionsandnonconformancesareidentified,correctedanddocumented.Theprocedurewillrequirearesponsefromtheaffectedpartyandwillalsorequireperiodicmanagementreviewofcorrectiveactions.

zr 2.GraverwillbesupplementingitsgQualityAssuranceactivitiesbyaddingapermanentQualityAssurancerepresentativetotheNineMilefieldcrew.InadditiontohisdefinedQualityAssuranceresponsibilities,hewillprovideanadditional~intertace>with<StoneandWebsterFieldQualityControl.~~~"<"~03.AsofJanuary16,1978,GraverhasassignedaQualityAssuranceManagementindividualtotheNineMilesiteonatemporarybasis.Thisindividualwillreviewtheactivitiesatthesiteand,reporttoGravermanagementontheprogressofQualityAssuranceandsiteactivities.4.TheappropriateConstructionInterfaceProcedurewillberevisedtoreflectaworkableInspectionReportandNonconformance&DispositionReportinterfacebetweenGraverandStoneandWebster.StoneandWebsterFieldQualityControlwillincreasetheirsurveillanceoftheGraverQualityAssuranceProgramandreporttoStoneandWebsterProjectManagementandNiagaraMohawkontheprogress,andeffectivenessofGraver'sQualityAssurancesiteactivities.
