FEEDBACK REPORT TO ALLEGER A-49 On November 14 and 15, 1984, Alleger A-49 was interviewed by TRT members Bob Hubbard, Shou Hou, Robert Masterson, James Malonson, Charles Richard, Charles Haughney, and John Zudans. The interview covered AQW-77, AQW-78, AQH-22, and an unnumbered issue concerning signing off on an NCR prior to the l
work being stafsfactorily completed.
Bob Masterson presented the TRT's' findings on A0W-77, concerning lack of weld inspection at the RHR heat exchanger tube supports. The TRT wassable to substantiate this issue; however, it concerned the containment spray heat exchangers rather than the alleged RHR heat exchangers.
Bob Hubbard presented the TRT's findings on AQW-78, concerning incorrect welding documentation of the oxygen analyzer. The TRT investigation showed the issue record card on the oxygen analyzer was not required to be kept as a permanent record by either the ANSI specification or Brown and Root procedures.
The issue record card needs to be kept only until the instrument is recalibrated; then, it may be discarded.
The TRT was unable to substantiate this issue.
The TRT attempted to discuss its investigative results on an issue concerning the fuel pool liner plate weld seams not matching the drawing location on the floor around the unit one reactor vessel.
The alleger denied this issue was his; the TRT will try to locate the rightful owner of this issue.
Jim Malonson presented the TRT's findings on AQH-22, concerning lack of hanger material traceability due to an incorrect heat number being used on a piece of hanger material. By logical reasoning and calling NPSI (the manufacturer),
the proper heat number was determined. The TRT was unable to substantiate this issue.
The alleger asked for the status of his issue concerning signing off on an NCR prior to the work being satisfactorily completed. The TRT was unaware of this issue and was unable to discuss it at this time. The TRT promised to pursue this issue and get the investigative results back to the alleger either by phone or latter.
