Weil, Investigator SUDJECT: VANDALISM - FILINGS IN SCRAM DISCHARGE VOLUME PIPE RECEIPT OF INFORIULTION On December 8,1981, Region III was informed by the Cleveland E1cetric Illuminating Company (CEI) that during flushing operations on December 1, 1981, a handful of metal filings was discovered in the scram discharge volume piping.

REGIO'I III INVESTICATION On December 8, 1981, the RIII Division of Resident and Project Inspectinn apprised the Region III Enforcement and Investigation Staff of the above information. A Region III investigator, who was at the site on another matter, was asked to obtain copies of any licensee memoranda concerning this matter. On December 10, 1981, the following memoranda were obtained from Ronald L. Farrell,Ifanager of Nuclear Quality Assurance at the Perry site: .u l 1. Untitled memorandum, dated December 2,1981, from D. Plunkett (Nuclear Test Section) to Hilliam Uright (Supervisor of the Nuclear Test Sec-tion). This memo concerned Spool No. 1-C11-G-IIC-86-RB of the scram

discharge volume piping. On December 1, 1981, the Nuclear Test Section found the spool's plastic cove
- had been cut and iron filings

. were inside the spool. The spool was cleaned, dried and rescaled.

l The Nucicar Test Section estimated the incident took place between 8:00 p.m., November 30, 1981, and 4:00 p.m., December 1, 1981.

2. Memorandum concerning the " Intentional Contamination of Piping",

f dated December 7, 1981, from Thomas E. Mahon, Supervisor of Plant Security to John J. Waldron, Manager of the Perry plant (Attachment 2).

Mahon's memo reiterated the details of Plunkett's memo.

i On January 14, 1982, Daniel R. Plunkett,.CEI Test Support Engineer-Mechani-cal Flush Section, provided the following information:

1. The spool section had not been welded. On November 30, 1981, the Mechanical Flush Section hydrolased Spool No. 1-C11-G-HC-86-RB once before the end of their shift. The ends of the spool were l covered with plastic at the end of the hydrolasing pass. On ,

December 1, 1981, the Mechanical Flush Section was scheduled to

  • l complete the hydrolasing of the spool piece and discovered the
plastic covering had been cut. Plunkett described the cut, as a t [

knife cut, several inches long. The plastic cover was removed and

iron filings were found inside the spool piece.

2. Plunkett advised the spool piece was designed to be a section of j a larger pipe discharging into the radwaste tank, and the only I

conceivable problem would be the jamming of a drain valve.had the iron filings not been found. Plunkett was of the opinion that -

the iron filings would not have caused any significant problems ,

as the filings would have been found during preoperational testing.

j Plunkett described the preventive measures taken as a result of this incident as:

1. All spool pieces were reinspected and none were found to have been vandalized.

j 2. The ends of the spool pieces were covered with tamper-proof seals. .

3. Security patrols in the area were increased.

Additionally, Plunkett advised the iron filings inside Spool No. 1-C11-G-HC- l l 86-RB was the only act of vandalism involving systems under.the responsibility j of the Mechanical Flush Section.  ;

REGION III INVESTIGATION STAFF ACTION t

On December 14, 1981, copies of the two memoranda were provided to i L. G. McGregor, Region III Project Inspector for the Perry Nuclear Power Project. On December 15, 1981, copies of the two memoranda were provided i to J. R. Creed, Chief, Safeguards Section. f On January 18, 1982, McGregor advised similar incidents had not occurred I at the Perry site, and the Division of Resident and Project Inspection i had sufficient information in this matter. Additionally, McGregor advised '

. t. . reck S210 pnant noos SUuJcCT On November 23, 1981, NTS accepted interim release number 743, which gave us jurisdiction of the scram discharge volume piping at the 90 area within the containment. This release was reciuested to allow the precleaning of subject piping. During the hydrolasing ef f ort on December 1,1981, the test support engineer noted that spool number 1-C11-G-HC-86-RB had been entered by cutting the plastic cover. This spool had been precleaned on November 30, 1981. t Upon further inspection of spool #86, iron filings were found inside the spool. The test support engineer directed that this spool be recleaned and the effluent filtered through a pillowcase.

This sample produced the equivalent of about one handful of iron filings. The spool was subsequently dried and re-sealed.

To date, all preeleaning has been accomplished on second shif t.

The mechanical / flush hydrolasing crew has been working on the C11 prec1 caning during the first four hours .of the second shif t to preclude interfering with scheduled work in the area. The re-contamination of spool #86 could have taken place anytime between 8:00 P.M. (11/30/81) and 4:00 P.M. (12/1/81), but'it is r'easonable

to suspect the period between 8:00 P.M. and 12 (midnite) on November 30, 1981. - -

' Letween 8:00 p.m. on 11/30/81 and 4:00 p.m. on 12/1/81, a person or persons unknown contaminated the scram discharge volume piping spool number 1-Cll-C-HE-86 RB, located at the 90 area within containment. The contamination was accomplished by cutting the

}i,, plastic spool cover and depositing metal filings within the piping.

G 1 On 12/4/81, met with W. Wright of N.T.S. who related that the 1 contamination of spool #86 did not appear to be the type of random i a

j vandalism previously experienced, but could be a sophisticated act j

of vandalism that would, if undetected, have serious consequences, To insure against a repeat of this act, the spools are to be scaled with a tamper indicating seal.

f An inspection of the area where spool #86 is located revealed that ij the contamination could have been acccmplished during normal work-i

" ing hours , however, the on going work and the volume of traffic through the parsage-way where the spool is located would make this F

4 difficult and risky for the perpetrator. It is more likely to 4 have occured during a period of low activity in'the area such as lunch break or during the back shifts. -

' Beyond this single act is the possibility that a knowledgeable 1 person or persons may be determined to vandalize equipment in a manner that could have serious implications. Individuals charged with the inspection and test of critical systems should be alert i

The Perry facility is under construction. Unit 1 is approximately 70 percent completed.

This Preliminary Notification is issued for information only.

Neither the licensee nor Region III is planning a press release.

The State of Ohio is being notified.

Region III (Chicago) was notified of the event by the licensee at approximately 1:30 p.m.

on December 8, 1981.

50-441 Site Area Emergency Perry, OH 44081 General Emergency vyy Not Applicable Subj ect: QUALITY CONTROL ELECTRICAL INSPECTOR FIRED Region III was informed on March 31, 1982 by the licensee that a Quality Control Inspector with Kaiser Engineers, Inc., has been fired from his job at the Perry plant following discovery that he had falsified information on his employment application.

The employee, who had been hired April 12, 1981, was an electrical Q.C. inspector at the time of his termination. Since Fegion III has been conducting an investigation into allegations involving electrical areas at Perry, the firing of the Q.C. inspector may l

result in media interest. The individual has not alleged any discrimination concerns with regard to his being fired.

Neither the licensee nor Region III plans to issue a news release. Region III will respond to inquiries.

The State of Ohio will be notified. This information is current as of 11:00 a.m.
