When an individual is contacted, read the infor-mation on Attachment 1.1-1 and instruct him/her to stand by for possible activation.
3. If the regular telephone system is not available for notifi-cation, use the pager/ radio to contact those personnel with pagers and radios. For those personnel without pagers / radios, dispatch a messenger with instructions to deliver a copy of

9. Notify the personnel listed on Attachment 1.1-2 if and den the Alert is teminated.

If the primary has not been reached, con-tact the alternates. then an individual is contacted, read the information on Attachment 1.2-1 and instruct him/her to imediately report to the Peabody EDC.
3. If the regular telephone system is not available for notifi-cation, use the pager/ radio to contact those personnel with pagers and radios. For those personnel without pagers / radios, dispatch a messenger with instructions to deliver a copy of Attachment 1.2-1 and to direct emergency response personnel to establish contact with the dispatcher at once, and then imediately proceed to the EOC. 9. Resune normal public safety dispatch functions once the Seabrook emergency-related dispatch activities have been fully transferred to the EDC.
10. Provide standby / backup assistance in the event of emergency connunications overload or systems failure at the EOC.
11. Unless specially called upon by the conmunications Officer, no further action is required.

, 6. Verify that the Police Dispatcher has succeeded in notifying all EOC Officers and other key personnel (or their alter-nates) to stand by. Refer to Attachment 2.1-1.

7. If verification reveals that alternate personnel are on standby status because of the unavailability of EOC

8. If for any reason the contacts for Northshore Mall are una-vallable, instruct the Police Dispatcher to notify the pd Mall's Security Department as a precautionary measure. The Mall Security Department should be directed to contact the most senior ranking mall official available, dio in turn should confirm the notification by telephoning the Police Dispatcher.
9. Advise MCDA/0EP Area I of any unusual conditions affecting the City such as: severe weather, catastrophic accidents, major traffic tie-ups, or major city-wide social events (parades , fairs etc.).
10. !!bsure radio contact is maintained with MCDA/0EP, Area I in order to receive follow-up information pertaining to the ongoing radiological emergency and it's prognosis for esca-lation or de-escalation.
11. If the Police Dispatcher reports an inoperable radio channel or any other emergency communications systen deficiencies, notify the Caussunications Officer at once.
12. Keep staff advised of important developments. Refer to Attachnent 2.1-2 for telephone listings. .
13. Review your procedures for Site Area Bnergency. Turn to Procedure 2.1.2.

6. Verify that the Police Dispatcher has succeeded in notifying all EOC Officers and other key personnel (or their alter-nates) to stand by. Refer to Attachment 2.2-1.
7. Notify, or direct the Police Dispatcher to notify, Peabody Schcol Officials of the radiological emergency in progress and of the need to prepare for possible early dismissal of classes . If the emergency occurs dien schools are closed, alert the Superintendent to assure ready access to school facilities. Refer to Attachnent 2.2.2.
8. As other members of the EOC staff arrive, instruct them to begin implementing their procedures for Site Area Bnergency.
9. If Alternate personnel report to the EC in lieu of EOC Officers, ensure each has a copy of the Implementing Procedures . Answer questions if necessary.

10. If any officer of the EDC staff is delayed in arriving at O the EOC, assign the implementing procedure of the absent officer to another member of the staff.

accidents, etc.

14. Request status reports frca all EDC Officers. Ensure all EOC Officers have completed their actions for Site Area O Energency.
15. Brief the Mayor on current event status, any actions int-tiated by the EOC, and the overall level of preparedness of the Peabody emergency response organization.
16. Issue / approve news /information statements concerning local activities. All rolesses must be authorized by the Mayor and coordinated with the Seabrook Media Center.

'17. Review actual local road and weather conditions with the Police Ottef and the Public Works Director to ensure that construction, flooding, ice, snow or tutusual traffic genera-

. tion will not hinder movement of personnel or vehicles during any peak evacuation period. Inform MCDA/0EP Area I of any urmet needs.

APPENDIX 8-A RADIGIIXIICAL EQJIPMENT INVENIORY ACTIONS

1. Verify with the Civil Defense Officer that the ntsnber of items required, as listed in TAB 1, Radiological Equipment Inventory, are accurate.
2. Record any changes in estimates for required equipment in the appropriate coltsun of TAB 1.
3. Comt the number of each it s listed on TAB 1 in the custody of the City of Peabody.
4. Perform operational checks on those items so designated by Figure
1. Instructions on how to perform the checks are provided as follows:

1. To check the Dosimeter Charger, loosen thumbscrew in the top or bottom center of the charger with a coin, such as a dime, and remove bottom case.

Install battery (in correct way, + and -) and reassemble.

2. Position the charger on a flat surface such as a table. Unscrew the cap on ,

3. Apply downwaro pressure and you should see a meter scale and a vertical line while looking through the dosimeter. If no line is visible, rotate the control knob, located in the upper right hand corner (Figure 1), until a line appears.
4. Set line to or notr zero (Figure 2) by turning control knob (Figure 1).

5. The charger is considered operational if the light source for reading dost-O meters is working and the charger can move the hairline on a self-reading dosimeter to, or close to, zero.
6. If the light source fails to work, replace battery and repeat check sequence. If light still fails to operate, replace the light bulb with the spare provided inside the charger case and repeat check sequence.
7. If the light source works but you are unable to move the line on the dosi-meter, clean the charging contact on the charger by rubbing with a pencil eraser and repeat the check sequence.

1. Visually check the meter for signs of physical damage.
2. Ensure the selector switch is in the "off" position.
3. Open case and install batteries. Return insertament to case.
4. Turn the selector switch to the "X10" position.
5. Connect the headphones to the audio jack.
6. Open the probe shield and put on the headphone.

Note: ENSURE 1HE CW-700 HAS BEEN All0WED TO WARM UP EUR AT LEAST 30 SECGIDS BEFORE BEGINNING SIEP 7.

7. Hold the probe's open window area against the operational check source on the I

side of CW-700. The meter should read between 1.5 and 2.5 mR/hr. An i

increase in the rate of clicks should be heard in the headphones.

8. If the meter reads too low, install new batteries and re-check the instru-ment. If no clicks are audible in the headphones, replace the headphones and recheck the insertment.

d. I Dosimetry-KI Report Form (TAB 1)

Each radiological monitor receives one unit as described above.

2. Have the radiological monitor complete the top section of the Dosimetry-KI Report Form as shown in Figure 1.

workers and supervisors are cautioned that the 5 Roentgen l figure is a guide and should attempt to keep exposure as low as reasonably achievable. The exposure to radiation should be kept to a mininus for all persons. Any one in-dividual should not receive a total dose far in excess of other energency workers if circumstances permit substitu-tion of personnel, termination of assignment or other protective action. If your dosimeter indicates an exposure of SR or greater, notify your supervisor and report immediately to the location specified.

d. Termination of Assigmaanc (1) Unless directed otherwise by your supervisor, at the end of your assignment, report back to your duty station.

Record the final reading of your dosimeter in the after block on the Dosimetry-KI Report Form. Subtract the before reading from the after reading and record results in the mission total block. Report mission completion and the total mission exposure to your supervisor. -

(2) If you are being relieved of your assignment by another individual, then:

I MCNITORING/DEC0erLWINATION AREA O 1. - Base procedures are designed to establish standard operating procedures for the City of Peabody monitoring and decontanination teams. May conform to the guidelines established by the Federal Bnergency Management Agency, the U.S. Nuclear Regulatory Commission, the Massachusetts Civil Defense Agency / Office of Bnergency Preparedness, the Massachusetts Department of Public Health and the City of Peabody Radiological Bnergency Response Plan. II. DEFINITIONS A. Decontamination Monitoring - the process of checking a person or object with a survey meter to ascertain if the person or object is con-taminated with a radioactive substance. [ B. Dosimeter Qurger - CD V 750; a device used to zero civil defense self-reading dosimeters dich in this plan are the CD V 138, CDV-730 and CD V 742. C. Dosimetry - Devices that measure or record exposure to radiation. D. Bnergency Classification IAvel - Four levels of emergency conditions - defined by the U.S. Nuclear Regulatory Commission.

1. INUSUAL EVENT: Events are in progress which indicate a potential degradation of the level of safety of the plant. No releases of

_O radioactive material requiring off-site response or monitoring are expected unless further degradation of safety systems occurs.

2. AIEE: Events are in progress sich involve an actual or potential substantial degradation of the level of safety of the plant. Any releases are expected to be limited to small fractions of EPZ Protective Action Guide exposure levels.
3. SITE-ARIA ENERGENCY: Events are in progress t.hich involve actual or likely major failures of plant functions needed for protection of the public. Any releases are not expected to exceed EPA Protective Action Guide exposure levels except near site boundary.
4. GBERAL DERGENCY: Events are in progress which involve actual or inusinent substantial core degradation or melting with potential for loss of containment integrity. 1. If not done previously, inventory and operationally check equipnent in accordance with Procedure 1.
2. Set up interior of area as shown in TAB A.
a. Remove any unnecessary equipment or material from the controlled area.
b. Cover floor surfaces with plastic sheeting. Secure plastic to the floor with duct tape.
c. Cover the plastic with paper or similar material in areas where personnel traffic is expected. This will reduce the slip hazard caused by the plastic.

d. Cover all equipment or devices in the controlled area not needed for your operations.
e. Arrange contaminated waste containers as shown in drawing. Line each container with a plastic trash bag.
f. Construct step-off pad by taping a large plastic trash bag to the floor. Cover this by taping two layers of paper over trash bag.
g. Arrange itens such as towels, soap, paper coveralls, forms, etc., in such a way that they are readily available.
h. Line the 2W gallon bucket with a plastic trash bag, then fill half the bucket with kitty litter. Place bucket in decontamination section of controlled area.
3. Prepare exterior of Decontamination Center as shown in TAB B.
a. Rope off or mark traffic flow into station. This includes marking off a vehicle monitoring area, a clean vehicle parking area and a contaminated vehicle parking area.

2. Place the probe in a light plastic bag to prevent the probe from becoming contaminated during personnel surveys.
3. Turn the CDV-700 range selector switch to the XI position.
4. With the probe window closed, determine background radiation level. If the background radiation is greater than 200 cpe, personnel surveys cannot be perfonned. To lower background radiation level:
a. Iocate the source of the background radiation by using the CDV-700.
b. Remove or shield the source of the radiation.
c. If the source cannot be located or reduced to a lower level, contact the -

Radiological Officer for assistance.

5. Open the window on the CDV-700 probe and put on the headphones.
6. Ehsure that the range switch is in the XI position.

In these cases the " hot spots" should be pre-washed with soap and water followed by a complete shower.

3. Have the individual move to the decontamination section of the controlled area.

a. For nonessential vehicles, hold vehicles in the contaninated vehicle parking area until assistance is obtained form NIAT team.
b. Ebr essential vehicles, three additional sprayings and scrubbings may be attempted and scrubbings may be attempted at the descretion of the Station Supervisor. If the vehicle is not decontaminated after three attempts the vehicle will be parked in the contaminated vehicle area until assistance from the NIAT is available. '

PURPOSE he se of this ywcedure is to outline the actions necessary to est lish and operate a reception center for the City of Peabody. %is procedure will ensure that evacuee resident and transient populations from Salisbury, Newburyport and Newbury receive appropriate information and directions to mass care centers and/or radiological monitoring station. Maintaining an evacuse census as well as evacuee referral to medical faci-lities or other needed services is also addressed herein. II. SITUATION A. here are two (2) reception centers (located in Peabody and N. Andover) l located along main evacuation routes in Massachusetts. B. It is estimated that up to 28,619 resident individuals could be eva-cuating out of the affected area and reporting to the Peshrvly reception center. As many as 20% or 5,724 may require mass care and/or radiolo-gical monitoring / decontamination.* . C. It is estimated thac up to 34,667 tourists and EPZ employees who live outside i the EPZ could be evacuating out of the affected area and reporting to ( the Peabody reception center. As many as 20% or 6,933 may require radiological monitoring / decontamination. Tourists and EPZ non-resident employees are not expected to require mass care acconmodations.* D. As evacuees arrive at the reception center, they will be evaluated as to mass care and radiological monitoring needs, assigned to appropriate mass care ac< =rviations and/or the radiological monitoring station and provided with strip maps to these facilities. E. An accurate record of vehicles processed and mass care center assign-ments will be maintained and monitored to ensure mass care centers are not filled beyond capacity. 5. Briefing of reception center staff to include assigmnent of respon-sibilities in the areas of:
a. Communications (Mall Security and RACES)
b. Traffic control / vehicle census (Security)
c. Strip map distribution for mass care center and radiological

a. Designate personnel to load reception center equipment which is stored at the Public Services Garage and deliver to the Reception Center (Attachment 12.3-4).
b. Deliver traffic control point equipment in accordance with Attachment 12.3-1 and as directed by the Police Otief.
c. Maintain a log of traffic control points to which equipment has been delivered by Public Services person-nel (Attachment 12.3-3).
d. Assess the road conditions of the evacuation routes.

Assign crews to remove any traffic impediments (e.g. < traffic accidents, road construction, vehicle break-downs, etc.) along any of the routes. If the route cannot be cleared, discuss with the Police Otief alter-nate routes dtich can be used for evacuation.

11. If you leave the EOC, assign an alternate to staff your position. Notify the Civil Defense Director of the change. -
12. At the end of the emert;ency, provide copies of your log and messages to the Civil Defense Director.
