This is an update to a previous report submitted on September 26, 1983. The event for which the report was submitted is described in the following paragraphs.

On September 12, 1983, maintenance personnel performing routine work noted that two fire rated penetrations in the Control Building at 111 feet elevation were unsealed. Fire watches were established in accordance with Technical Specification 3.7.7 upon notification to Operations. _ Material Non-Conformance Report (MNCR) Number 892-83 was issued to track correction of the deficiency.

Penetration CE428B, in the Control Building / Turbine Building wall at 111 feet elevation, was verified closed during the Control Building penetration walkdown performed from December 10, 1982,- to February 24, 1983. The penetration was apparently opened during more recent maintenance work. Penetration CE77E is a Control Room floor. penetration in panel P739.

Programmatic controls, to require notification of and authorization by the Shift Superintendent / Supervisor prior to opening a penetration, were implemented.

The penetrations were resealed and the Limiting Conditions for Operation (LCOs) were lifted on September 17, 1983. This is submitted as a final report.
