This is an update to a previous report submitted on September 20, 1983. The event for which the report was submitted is described in the following paragraphs.

On September 8, 1983, during preparation to repair a C09leak downstream of valve P64F467, the valve was found already closed. The valve must be open to supply operating pressure to the CO2 fire protection master selector valve. The master selector valve IF373) is required to be operable for the CO t discharge from the tank to the main hrader. The 2

closed valve isolated all CO fire Protection. The valve was reopened 2

after discovery.

The valve was inadvertently closed. It could not been determined who closed the valve or.why it was closed. The maintenance work authoriza-tion to repair the leak was initiated on August 5,1983. The mechanic surveying the job scope around August 7 noted the leak was still present (valve was open). In addition, it is known that the valve was open on August 16 when a fuse supplying the master selector valve blew causing C09 to discharge into the main header. Therefore, the valve was apparently closed sometime between August 16 and September 8,1983.

Further investigation revealed that the present piping and instrumenta-tion drawings did not reflect the correlation between valve F467 and the selector valve, F373. However, a design change (DCR 83-394) had been requested to show the valve locked open and to reflect the as built condition. Also, the System Operating Instruction (04-1-01-P64-3) and the Valve Lineup Verification Surveillance (06-0P-SP64-M-0011) did not include valve F467 in the valve lineups. The system was reviewed for similar deficiencies which could cause isolation. None were found.

The procedures were revised to require the valve to be locked open. All outstanding design changes which were initiated to reflect the as-built condition of the plant have been reviewed for their affect on system operating instructions. Affected instructions were revised accordingly.

The event for which the report was submitted occurred on September 8, 1983, when an operator discovered the valve which supplies operating pressure to the CO fire Protection master selector valve closed.

2 The selector valve cust be operable to allow CO to discharge from the tank to 2

the main header. The closed valve isolated the CO fire Protection system 2

which is required to be operable by Technical Specification 3.7.6.3. This was reported pursuant to Technical Specification 6.9.1.12.b.

Corrective action for the above event is complete. This is a final report. Attached is LER 83-122/01 X-1 with Supplementary Information.
