Technical Specification Surveillance 4.3.3.9.1.b. requires the performance of this functional test every 31 days. The root cause of this event has been attributed to inadequate adminstrative controls associated with the implementation of the new computer based work activity system, MMIS. Technical Specification surveillance 4.3.3.9.1.b was not scheduled and performed as required due to a misinterpretation of the scheduling responsibilities.- The required functional surveillances were succesfully completed on 12/23/87. A review of Chemistry Department Technical Specification required surveillances, during the period in question, was conducted. No additional missed surveillances were discovered. This event wili be reviewed with all Chemistry Department supervision by Chemistry Department management. Administrative concerns associated with the implementation of the new MMIS system are being reviewed and addressed. The corrective actions associated with these administrative concerns will prevent ~ecurrence of this type event.

Technical Specification Functional Surveillance 4.3.3.9.1.b Performed Late Due to Inadequate Administrative Controls Discovery Date: 12/23/87 Report Date: 1/22/88 This report was initiated by Incident Report No.87-504.

CONDITIONS PRIOR TO OCCURRENCE:

Unit 1: Mode 5 Reactor Power 0% - Unit Load 0 MWe Unit 2: Mode 1 Reactor Power 100% - Unit Load 1150 MWe DESCRIPTION OF OCCURRENCE:

On December 23, 1987 at 0825 hours0.00955 days <br />0.229 hours <br />0.00136 weeks <br />3.139125e-4 months <br />, it was discovered that the required functional test for both Unit 1 and Unit 2 Waste Gas Oxygen Monitors were not performed within their required time frame.

Technical Specification Surveillance 4.3.3.9.1.b. requires the performance of this functional test every 31 days.

The Unit 1 and Unit 2 monitors' functional surveillances were required to be done as of November 8, 1987 and December 6, 1987, respectively. The Unit 1 and Unit 2 monitors' functional surveillances were completed on December 23, 1987. The Unit 1 November surveillance functional was not performed since the monitor required maintenance worke The Unit 1 and Unit 2 waste gas monitoring systems are cross connected such that either Unit's monitor will satisfy the Technical Specification monitoring requirements. Therefore, the Unit 2 monitor satisfied the November monitoring requirements for Unit 1.

APPARENT CAUSE OF OCCURRENCE:

The root cause of this event has been attributed to inadequate administrative controls associated with the new computer based work activity system, Managed Maintenance Information System (MMIS).

Technical Specification surveillance 4.3.3.9.1.b was not scheduled and performed as required due to a misinterpretation of the scheduling responsibilities.

Prior to the September 22, 1987 issuance of revision 8 to station Administrative Procedure AP-12, "Technical Specification Surveillance Program", Chemistry surveillances were scheduled by an internal Chemistry Department system. When the transition was made to fully

In March 1987, the Station Planning Department began issuing "Daily Scheduling Reports" which identified those work activities (including surveillances) which were due within the following three days. This report was issued as an interdepartmental aid for scheduling work *

. Chemistry supervision used this report for scheduling this surveillance, subsequently dispencing with their internal tracking system.* At the on-set of the Unit 1 refueling outage, Station Planning replaced this report with a report which only addressed the Unit 1 refueling outage related activities. Chemistry supervision subsequently missed the oxygen monitor functional surveillance.

ANALYSIS OF OCCURRENCE:

The Waste Gas Holdup System oxygen monitors are used to identify increased ievels of oxygen before a potentially explosive mixture develops. Contained within the Waste Gas Holdup System is hydrogen.

Hydrogen enters the system as a result of normal plant operations.

The mixture of hydrogen with sufficient levels of oxygen is explosive.

The periodic functional test of the oxygen monitors ensures their continued acceptable operation. The late functional surveillance was successful. No corrective maintenance was required based upon the results of the functional test. No problems with the Unit 2 Waste Gas Oxygen Monitor were indicated by a review of the Incident Reports for the period between December 6, 1987 and December 23, 1987.

However, this event is reportable to the Nuclear Regulatory Commission in accordance with 10CFR 50.73(a) (2) (i) (B) since the monitors were not surveilled within the required time frame of Technical Specification 4.3.3.9.1.b.

CORRECTIVE ACTION:

The required functional surveillances for both Units' monitors was successfully completed on December 23, 1987.

A review of all Chemistry Department Technical Specification required surveillances, during the period when Station Planning was not providing the "Daily Scheduling Report", was conducted. No other missed surveiilances were discovered. Therefore, this event is considered an isolated occurrence.

This event will be reviewed with all Chemistry Department supervision by Chemistry Department management.
