On May 14, 1996 during a NRC Licensing and Design Basis Co.nf ormance Inspection,.

Technical Specification Surveillance 4.9.12.d.2 was identified as not being implemented and this constituted a missed surveillance. Surveillance 4.9.12.d.2 is to be performed at least once per 18 months by "verifying that on a high radiation test signal, the Fuel Handling Building (FHB) Ventilation system automatically starts (unless already operating) and directs its exhaust flow through the HEPA filters and charcoal adsorber banks.

,, The FHB Ventilation auto start was never installed in Salem Unit 2, is not described in the UFSAR, nor does the *accident analysis take credit for or acknowledge this feature.

The cause of the Technical Specifications containing the FHB Ventilation auto start surveillance is attributed to a lack of adequate controls for the development and maintenance of Technical Specification surveillance procedures.

Corrective actions *include installation of the FHB Ventilation System auto start, and the initiation of the Technical Specification Surveillance Improvement Project.

This event is re~ortable in accordance with 10 CFR 50.73(a) ( 2) ( i) ( B) I any condition prohibited by the plant's Technical Specifications.

CONDITIONS PRIOR TO OCCURRENCE At the time of identification, Salem Unit 2 was shutdown and defueled. The Technical Specification surveillance Mode Applicability is whenever irradiated fuel is in the storage pool.

DESCRIPTION OF OCCURRENCE On May 14, 1996 during a NRC Licensing and Design Basis Conformance Inspection, Technical Specification Surveillance 4.9.12.d.2 was identified as not being implemented and this constituted a missed surveillance. surveillance 4;9.12.d.2 is to be performed at least once per 18 months by "verifying that on a high radiation test signal, the Fuel Handling Building (FHB) Ventilation system automatically starts (unless already operating) and directs its exhaust flow through the HEPA filters and charcoal adsorber banks." This had previously been identified within the corrective action program in October 1995. At that time an erroneous determination was made that this was not a Technical Specification violation and therefore not reportable.

The FHB Ventilation System auto-start was never installed in Salem Unit 2, is not described in the UFSAR, nor does the accident analysis take credit for or acknowledge this feature. The original Salem Unit 2 Technical Specifications were developed from the Westinghouse Standard Technical Specifications, NUREG-0452 .. The wording in NUREG-0452 is identical to that currently contained in the Salem Unit 2 Technical Specifications. Apparently, during the development of the Salem Unit 2 Technical Specifications, the preparer did not verify that FHB Ventilation auto-start was not a design feature for Salem Unit 2. Salem Unit 1 does not have an installed FHB Ventilation System auto start and the Unit 1 Technical Specifications do not contain the same surveillance as Unit 2.

CAUSE OF OCCURRENCE The cause of the Technical Specifications containing the FHB Ventilation auto start surveillance is attributed to a lack of adequate controls for the development and maintenance of Technical Specification surveillance procedures.

PRIOR SIMILAR OCCURRENCES The Technical Specification Surveillance Improvement Project (TSSIP), along with other LERs, has identified surveillance procedure issues. However, there are no other known instances where there are surveillances for equipment that is not installed in the plant.

SAFETY CONSEQUENCES AND IMPLICATIONS There were no safety consequences for this occurrence since the design of the plant did not include the FHB Ventilation auto start, nor ~as it described in the design basis documents. In addition, there are procedural.controls for the FHB Ventilation System to be in operation prior to the movement of fuel in the building. Therefore, not having the FHB Ventilation aut~ start did not have any impact on the operation of the plant. The health and safety of the public were not affected by this occurrence.

CORRECTIVE ACTIONS

1. The auto-start capability is being added to the Salem Unit 2 FHB Vent~lation system and* will be completed prior to .fuel movement.
