Purpose

This notice is being issued to ensure that recipients

are aware of the correct commercial

telephone

numbers for the NRC that are to be used to report inci-dents and reportable

items as required by the regulations

and license condi-tions. In addition, it is considered

prudent that recipients

assure-that

other telephone

numbers used for emergency

notifications

are correct, and provisions

exist for periodic updating.It is expected that recipients

will review and forward the information

in this notice to cognizant

individuals

for review and appropriate

use. Suggestions

contained

in this information

notice do not constitute

NRC requirements;

therefore, no specific action or written response is required.Description

of Circumstances:

As a result of the incident at the Sequoyah Fuels Corporation

on January 4, 1986, it was determined

that some telephone

numbers held by the licensee for emergency

notification

and assistance

were incorrect.

For exam-ple, the numbers for the NRC Region IV office and DOE emergency

radiological

assistance

were incorrect.

In addition, the DOE number was improperly

identi-fied in the radiological

contingency

plan under a listing for NRC. A prelimi-nary review of other contingency

plans indicated

that this situation

is not unique to Sequoyah Fuels Corporation.

Discussion:

The following

for notification

to the NRC of incidents

and reportable
